Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and issued under the authority of FAR Subpart 13.5 "Test Program for Certain Commercial Items," as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. EMV-2003-RQ-0033, issued as a Request for Quotation (RFQ) is for the purchase of various fiber cable & associated parts manufactured by Corning and Fluke Network. The solicitation document and incorporated provisions and clauses are those in effect thru Federal Acquisition Circular No. 2001-14, dtd 5/22/03. This procurement is issued on a non-restricted basis, NAICS Code 335929/small business size standard is 1,000 employees. Only one award will result from this solicitation via the issuance of a firm-fixed-price simplified acquisition order and/or a GSA Schedule Delivery Order (i.e., if it is determined that the issuance of both types of orders are necessary, only one Contractor will receive both awards since the Gov't requires a single source for all items). Items are req'd to be delivered, FOB Destination, to FEMA, Mt. Weather Emergency Operations Center, 19844 Blue Ridge Mountain Road, Bluemont, VA 20135, which is also the place of inspection & acceptance. MANDATORY: Successful Contractor must be capable of providing all the above items (ALL OR NONE), must provide the specified quantities/lengths (cable must be cut to the specified lengths), must provide the mfg & associated P/N listed for ea item. EVALUATION: The Gov't will make award on the basis of lowest overall evaluated price of quotes meeting or exceeding the acceptability standards for the non-cost factors (i.e., lowest cost/technically acceptable). In order to be technically acceptable, the Contractor must provide the items as specified and the Contractor must provide 3 references to support the firm's capability to delivery quality products in a timely manner (include a current point of contact with working telephone number for each reference so the Gov't can verify the information for acceptability). Offerors may provide pricing using GSA Schedule Contract pricing and/or open market pricing. Items offered under GSA Schedule pricing shall be so designated and the Offeror shall provide the GSA Schedule Contract info (i.e., contract number, discount terms, expiration date). For the open market items, offerors are to include a completed copy of FAR provision 52.212-3 "Offeror Representations and Certifications--Commercial Items" w/their response. The following FAR clauses also apply: 52.212-4 "Contract Terms and Conditions--Commercial Items," 52.212-5 "Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items," 52.203-6 "Restrictions on Subcontractor Sales to the Government," 52.219-8 "Utilization of Small Business Concerns," 52.222-21 "Prohibition of Segregated Facilities," 52.222-26 "Equal Opportunity," 52.222-35 "Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era," 52.222-36 "Affirmative Action for Workers with Disabilities," 52.222-37 "Employment Reports on Disabled Veterans and Veterans of the Vietnam Era," 52.222-19 "Child Labor--Cooperation with Authorities and Remedies," and 52.225-1 "Buy American Act--Supplies," 52.232-34 "Payment by Electronic Funds Transfer--Other than Central Contractor Registration."
