Description
Visn 9 located at 3400 Lebanon Pike, Murfreesboro, TN 37129 is seeking services for providing dental care for veterans in the Chattanooga and Knoxville, TN areas. The provider must provide a fully equipped dental facility including but not limited to all labor supplies, equipment, components, dental hygienists, dental technicians. The VA will refer patients to the provider for dental care. Authorization for service will be submitted by the VA. Services authorized may include examinations, necessary radiographs, diagnostic procedures, prophylaxis, and any emergency type service. The initial visit will consist of examination, radiograph, diagnostic procedures, prophylaxis, treatment plan and emergencies. Emergency treatment will be dental procedures necessary to resolve symptoms of pain, infection and/or bleeding, possible examination and radiographs, incisions, extractions, sedative fillings, and pulpectomy. Any other services would require prior authorization. The authorization form will be Form l0-2570d(R) or an ADA approved form. An example of information we would need from the offeror is the date, tooth number, surface code as applicable, description of service, CDT4 procedure code, and contract fee. Once treatment is completed the date service was completed. Payment may be submitted as the procedures are completed during each treatment visit or in total at the completion of the treatment plan. Patient access to treatment must be accomplished within 30 workdays. Patient access to emergency dental care to is to be accomplished within 36 hours. Possible contract period will be one year with three one year renewal options. A copy of dental codes are attached which lists the projected quantity required per year.
