Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation will be issued as a Request For Quotation (RFQ) under the following solicitation number:FNW00000000013 (iii) This RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2001-14. (iv) This acquisition is set-aside 100% for small businesses in regards to size standards. The applicable NAICS is 562991, and the small business size standard is $6 Million. (v) This RFQ contains 9 line item(s): See below (xvii) for complete line item listing. (vii) The performance period will be from 0800 on August 9, 2003 until 1900 on August 10, 2003. Delivery location/Place of performance is Hickam AFB, Hawaii. Acceptance will be performed by the Government. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) Offers will be evaluated using the following evaluation factors: lowest price that meets the requirement. (x) All offers must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items. (See Attached Representations & Certifications) (xi) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items applies to this acquisition. 52.219-1 Alt.1, Small Business Program Representations (Apr 2002). The mens side will have 3 individual restroom stalls, 4 urinals with privacy screens dual sinks with mirrors, 1 hand towel receptacle, 2 soap dispensers, and 1 garbage bin 005Services of CLIN 001 (see info. below) 2 each daily 006 Services of CLIN 002 (see info. below) 2 daily 007 Services of CLIN 003 (see info. below) 2 daily 008 Services of CLIN 004 (see info. below) daily 009 Waterless hand-washing stations 30 each Additional information: -Portable toilets/urinal stations and hand-washing stations will be needed for Hickam?s Friend?s & Neighbors Weekend on August 09 and August 10, 2003. -All portable toilets/urinal stations and hand-washing stations must be delivered to Hickam AFB and located as directed by the Government no later than 0800 on August 09, 2003. -Basic services of CLINS 001, 002, 003, 004 consists of emptying each portable toilet/urinal station and restocking toilet tissue, twice daily, between the hours of 1200 and 1300, and 1700 and 1800. -Additional services of CLIN 004 will include an on-site attendant to service, supply, and maintain DV trailer throughout the day as required or directed. -Basic services for CLIN 009 consists of keeping waterless hand-washing stations filled throughout both days. -All portable toilets/urinal stations and hand-washing stations must be removed from Hickam AFB no later than 1600 on August 11, 2003
