- Contractor to provide all personnel, equipment, tools, materials, supervision, and other items necessary to furnish requirement for a PREVENTIVE MAINTENANCE SERVICES FOR FITNESS CENTER CARDIOVASCULAR AND WEIGHT TRAINING EQUIPMENT. This is a commercial acquisition. This solicitation is a request for quotation and is issued as a 100% small business set-aside under the Standard Industrial Classification code - 7623, NAICS code - 811310, and the related Small Business Standard - $6M. The base performance period is tentatively from 1 Oct 03 through 30 Sep 2004 with up to two (2) option year periods. All potential sources must obtain a DUNS number and register in the Central Contractor Register (CCR) in order to transact business with the Department of Defense (DoD). Failure to obtain a DUNS number and register in the CCR may result in the delay of award of a contract or possible award to the next otherwise successful offeror who is registered in the CCR at the time of award. Solicitation will be issued on or about 20 June 03. Due date will be stated in the solicitation. IAW FAR Part 5.101(2), ?All responsible sources may submit a response, which if timely received, must be considered by the agency.? Offers shall be evaluated in accordance with FAR 52.212-2, Evaluation -Commercial Items (Jan 1999), which is incorporated into this Request for Quote, with addendum to paragraph (a) as follows: The following factors shall be used to evaluate offers: (i) Past performance and (ii) price. The Government will make award to the responsible offeror whose offer conforms to the solicitation and is the greatest value to the Government. Past performance will be considered slightly more important than price. Each proposal shall provide recent and relevant information regarding an offerors action under previously awarded contracts. Offerors shall identify past or current contracts (including Federal, State, local government and commercial) for efforts similar in size, scope, type, and complexity to the requirements stated in this synopsis. Provide references for all recent and relevant work performed in the last three (3) years or not to exceed 4 projects. When past performances are evaluated essentially equal, price may be the deciding factor.
