- This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) sub part 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation and THERE WILL BE NO WRITTEN REQUEST FOR QUOTATION ISSUED. The solicitation number for this Request for Quotation is RFQ-797-FDF6-03-0043. This solicitation is 100% set aside for Small Business. This solicitation is issued on a restricted basis as a total small business set aside under NAICS Number 339112. DESCRIPTION: 4 EACH MICROLINK HEADLIGHT FIBER 12FT (3.8M) REF PART #90128, 4 EACH CL300 HIGH-OUTPUT HEADLIGHT ILLUMINATION SYSTEM INCLUDING: 90123 CL300 SURGICAL ILLUMINATOR 85-265VAC, 90124 CL SERIES SURGICAL ILLUMINATOR STAND, 49540-S HIGH OUTPUT MFI HEADLIGHT/FLEECE COMFORT HEADBAND, 90142 MICROLINK HIGH OUTPUT 3MM FUSED MFI FIBER, 90041 POWER CODR HOSPITAL GRADE 15 FT (4.8M) NORTH AMERICAN (THIS IS THE REPLACEMENT FOR THE COGENT XL), 4 EACH MICOLINK COAXIAL HEADLIGHT WITH SWEATBAND HEADBAND PART # 90126, 2 EACH CL300 LAMP FIXTURE (SOLARTEC 270 COMPATABLE) PART # 90125, 2 EACH TERRY CLOTH HEADBAND (10/BAG) PART # 90013, 1 EACH NYLON DRAWSTRING BASG (10/PACK) PART # 90161 OR EQUAL. Like items bids will provide specifications for evaluation by the end user. Bids are FOB destination to Keesler AFB MS 39534. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Clause 52.212-1, Instructions to Offeror-Commercial Items. As stated in this clause, the Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror_s initial offer should contain the offeror_s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussion if later determined by the Contracting Officer to be necessary. Offerors shall include a completed copy of provision 52.212-3, Offeror Representations and Certification--Commercial Items. An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition.
