- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are b eing requested and a written solicitation will not be issued. The solicitation is issued as a request for quotation (RFQ) DABJ05-03-T-0561. This notice is issued unrestricted NAICS 54519. The requirement is T&M for the procurement of Environmental Qualit y Report Program Operations and Maintenance (EQR) II. EQR is a web-based data collection application designed to provide an automated environmental reporting process to collect measures of merit (MOMs) data for service headquarters (HQ), Regions, Major C ommands and installations. Data is collected for compliance conservation and pollution prevention. A Statement of Work shall be provided by contacting the Contract Specialist listed below. The Period of Performance is for 12 months from date of award. Delivery point is Edgewood Area, Aberdeen Proving Ground, MD 21005-21010 at Destination. Offerors must submit a proposed delivery schedule for the requested requirement. Offerors must include a completed copy of the provision at Clause 52.21-3, Offeror R epresentations and CertificationsâCommercial Items. Applicable Clauses to this solicitation are 52.212-4, Contract Terms and ConditionsâCommercial Items, Clause 52.212-5, Contract Terms and conditions Required to Implement Statutes or Executive OrdersâCommercial Items. In accordance with Part 13.106, a description of the relative importance of evaluation factors is not required. Responses to this RFQ must be signed, dated, and received via electronic mail or FAX by June 12, 2003, no later than 4:00 pm EDST at the US Army Contracting Agency, SFCA-NR-APC-T (Jo Ann Hayes), 4118 Susquehanna Avenue, Aberdeen Proving Ground, MD 21005-30 13.
