Description
NA The Minneapolis VA Medical Center has a requirement to provide prostate cryotherapy services on an as needed basis for patients with prostate cancer who cannot tolerate chemo-therapy or brachy-therapy procedures. Contractor to provide prostate cryo-ablation services on site at the Minneapolis VA Medical Center. Contractor shall provide the following: 1. The cryo-ablation system including all associated disposables (cryo-needles, cryo-ablation thermal sensor needles, etc.) 2. Prostate procedure accessories, including a stepping and stablizer system and all associated disposables 3. the ultrasound scanner, including all accessories, transducers, etc. 4. System application specialist and sonographer for each case 5. The contractor will provide all maintenance, repairs and upgrades to the equipment. Note: The VAMC will provide medical gases as required (i.e. argon, helium, etc.). Payment will be made on a per case basis. 12-month periods. Total contract period not to exceed 36 months. Contractor shall be responsible for providing his/her own housing, transportation, malpractice insurance, professional membership fees, continuing education costs, sick leave and vacation time. All offers submitted will be referred to a Technical Evaluation Panel for review. Award of a contract will be based on the following: professional qualifications; clinical and consultation skills; availability; past experience and performance history; and total cost. Solicitations are being prepared under the authority of Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United State Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures. and FAR Part 15, Contracting by Negotiation. Solicitation documents will be available on or about June 18, 2003. Offers are due no later than COB on July 18, 2003.
