- Description
- Provide outpatient End-Stage Renal Disease (ESRD) dialysis services to eligible American Indians and Alaskan Natives who are Indian Health Service Contract Health Service beneficiaries. Services shall be provided at the Sells Service Unit Dialysis Center, Sells, AZ and the Pascua Yaqui Health Care Facility, Tucson, AZ. RFP release date will be on/about June 17, 2003. Requests for this solicitation may be made in writing or faxing request to 520/295-2626. Period of performance shall be effective one year from date of award plus four one-year option to extend.*****
