- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The North American Industrial Classification System (NAICS) code applicable to this solicitation is 339112, Surgical and Medical Instrument Manufacturing, and the small business size standard is 500 employees. This is a full and open competition (unrestricted). DESCRIPTION, QUANTITY, UNIT, UNIT PRICE, TOTAL: Bid Item 1: The VA Tennessee Valley Healthcare System intends to purchase a Medtronic Paceart 2000 System, brand name or equal. 1 UNIT $_______ . Unit price is to include shipping, set up installation, training, software, hardware, warranty, and support services necessary to obtain full operational value of Medtronic Paceart 2000 System or equal. System Software must include, two (2) module workstation license fee; one (1) data entry workstation license fee. Hardware for Workstation One is to include Paceart Hardware; Clinical Module(parallel port) with associated cables and programmer interface kit; Laptop computer; modem; network card; printer; WinFax Pro 10.0 fax; and MSDE Software; LapLink Software License. Hardware for Workstation Two is to include Paceart Hardware; Transtelephonic Module(parallel port) with associated cables; Desktop computer, modem, network card, tape back up; MSDE Software; Laplink Software license. Workstation Three is to include: desktop computer, modem, network card, MSDE Software; LapLink software license; Network Configuration; Network Type; Peer to Peer; Network Hub. Contractors quote is to include all set up, installation and training. Training to consist of a minimum of 2 days on-site. Warranty period of one year is required to cover software upgrades and all hardware. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses in effect through Federal Acquisition Circular 2001-12 and VA Acquisition Regulation (VAAR) clauses as follows: 52-211-6 Brand Name or Equal; 52.212-1 Instructions to Offerors Commercial Items; 52.212-3 Offeror Representations and Certifications Commercial items; 52.212-4 Contract Terms and Conditions Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items with (b)(11), (12), (13), (14), (15), (16), (18), (21) and (25) checked; and VAAR 852.270-4 Commercial Advertising. Offerors to include completed copy of 52.212-3 Offeror Representation and Certifications commercial items. The provision at 52.212-2, Evaluation of Commercial Items, applies to this acquisition. The evaluation criteria stated in paragraph (a) of this provision are as follows: (1) Must be compatible with the Medtronic Paceart System and (2) price. The system offered must meet all the salient characteristics specified above to be considered for award. Award will be made to the offeror proposing an acceptable system at the lowest overall price.
