- NA This is a combined synopsis/solicitation for commercial items prepared in accordance with the Federal Acquisition Regulation (FAR) subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only sol icitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quotation is DADA09-03-T-0391. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-13. This solicitation is 100% set aside for small business; the associated NAICS code is 339112; No. of employees: 500. This requirement is for the Department of Surgery, Brooke Army Medical Center, Fort Sam Houston, Te xas. ITEM DESCRIPTION: 0001 - Leksell Stereotactic System, Model G (Basic System) OR EQUAL; Includes Multi-purpose Arc with Axis and Sliding Carrier, LSS Frameholder Adapter; MR adaption Kit; CT adaption Kit; Basic Functional Instruments; Sedan Side-Cutt ing Biopsy Needle Kit II (Diameter 2.1mm, 4.1.2mm Window); Backlund Biopsy Kit, Complete (includes: A2400-01, A2420-01, A2410); Repeat Fixation Kit; X-Ray Indicator with Rulers for Analog X-Ray; Instrument Screw Driver, double ended; Socket Wrench for Quic k Fixation Screw, (2 ea); Quick Fixation Screw Set, 20 pairs (sizes: 20mm, 22.5mm, 25mm, 27.5mm, 30mm, 32.5mm, 35mm, 37.5mm, 40mm, 42.5mm, 45mm, 50mm, 55mm, 60mm, 65mm, 70mm, 75mm, 80mm, 90mm, 110mm); Hennig Instrument Guide; Application training for Henni g nstrument Guide; Installation & training, Case Support, Leksell Stereotactic System Instructions for Use Manual. System MUST interface perfectly with existing OR, MRI, CT, and data acquisition equipment currently being used at Brook Army Medical Center. If offering an equal item, provide complete technical documentation and specifications to be able to evaluate. Delivery, which shall be FOB Destination to Brooke Army Medical Center shall occur within 30 days after award of the purchase order. Offerors responding to this announcement shall provide all information contained in the Federal Acquisition Regulation (FAR) Provision 52.212-1, Instructions to Offeror-Commercial Items. As stated in this provision the Government intends to evaluate offers and awa rd a contract without discussions with offerors. Therefore, the offeror??????s initial offer should contain the offeror??????s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later de termined by the Contracting Officer to be necessary. 52.212-2, Evaluation -- Commercial Items.
