Description
Provided below are the Questions and Answers for S-LMAQM-03-R-0084 that were received before May 5, 2003 at 4:00 p.m. deadline. 1. Clarify the outguide clearance, is the existing clearance of 10.2? for media sufficient? Answer: Yes 2. How many cartons need to be interfiled/back shifted within units 9 & 10? Answer: 18 Boxes 3. How are proposal to be submitted? Answer: Proposals are to be submitted by mail, courier or hand delivery as follows: First Class Mailing:U.S. Department of State Department Office of Acquisition Management P.O. Myer Drive Arlington, VA 22209 Attn: Brian Carper 4. What are the rules of the building, what is the earliest in the afternoon the equipment can be moved? What is the availability of the Freight elevator? Answer: In addition to what I sent you a moment ago, the Lektrievers will have to be relocated after 5:00pm. 5. Confirm the hours of the loading dock coordinate with the hours the freight elevator is available. Answer: After 5:00 p.m. 6. Can the equipment be GSA and the moving portion be open market? Answer: A/LM will answer this question. 7. Will the revised scope of work indicate a minimum clear height of 10.5 inches to accommodate the out guide? Answer: No plans to revise SOW; current SOW stands. 8. Will the revised scope of work allow vendors to substitute a roll-out letter size drawer for letter trays? Answer: No plans to revise SOW 9. Will the procuring agency agree the data being stored is a paper medical record and is not electronic data? Answer: See Scope of Work. 10. Will all records need to be accessed at present location during the relocation project? Answer: Not all of the records. The electronic medical record will be used as much as possible. 11. What is the absolute minimum shelf clearance required in the new machines? Answer: The current clearance is the absolute minimum. 12. How will offeror?s submittals be verified to substantiate the shelf clearance of their machines? Answer: Offeror?s proposals are to adhere to the Statement of Work. 13. Is a signed RFQ Standard form 18 or other, required with the submission of the proposals? If so, please post a downloadable version of the form at Fedbizopps.gov. Answer: No, a Standard form 18 will not be required. 14. Corrected answer to question ? Please confirm the maximum height for the new units so not to interfere with mechanical infrastructure in the ceiling? Answer: The ceiling height was measured at 8 feet 2 inches (floor to ceiling grid). The maximum height available for new machines is 9 feet (108 inches).
