- Description
- This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for quotes (RFQ) are being requested and a written solicitation will not be issued. Solicitation documents, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular 2001-13. This procurement is unrestricted. The NAICS is 423450 with business size standard 100 employees. A firm-fixed price contract is anticipated. The South Texas Veterans Health Care System, Audie L. Murphy Division, 7400 Merton Minter Blvd., San Antonio, TX 78229 has a requirement to purchase for the Anesthesiology Svc.: five (5) Hemodynamic Monitor Assemblies, LiDCOplus part# HM71-02 manufactured by LiDCO or equal.=20 Each LiDCOplus consists of one (1) 15=94 Color LCD Touchscreens, one (1) Analogue Blood Pressure Inputs (0-2.5 VDC, 1v/100mm Hg), File downloads, System Software, and one (1)) Marquette Pressure Monitor Cable Connectors. All quotations shall include a trade-in allowance for four (4) PulseCO Continuous Hemodynamic Monitors, Mod#CM-71-01 and two (2) LiDCO Cardiac Output Calibrations units. Quotations shall include all warranty information, installation and technical data.=20 Quotations shall be submitted F.O.B. destination. All quotations are to include a completed copy of the provision at 52.212-3, Offer Representations and Certifications-Commercial Items, with quotation. (A copy of this provision may be requested by facsimile to (210) 617-5255, Attn: Jo Collins). The following provisions and clauses apply to this action: 52.212-4 Contract Terms and Conditions Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statutes and Executive Orders-Commercial Items including clauses incorporated by reference under Section b (11-16)(19)(21-22)(25), Section c (1-2), 52.212-1 Instruction to Offerors-Commercial Items, VAAR clause 852.211-77 Brand name or equal, 852.211-70 Requirements for Operating and Maintenance Manuals, 852.211-71 Guarantee Clause and 852.270-4 Commercial Advertising.
