- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Request for Quotes (RFQ) are being requested and a written solicitation will not be issued. Solicitation documents, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular 2001-13. This procurement is unrestricted. The NAICS is 421450 with business size standard 500 employees. A firm-fixed price contract is anticipated. The South Texas Veterans Health Care System (STVHCS), Audie L. Murphy Division, 7400 Merton Minter Blvd., San Antonio, TX 78229 has a requirement to purchase a Baylis PMG Model 115 generator, part number PMG115. Unit includes the following accessories; one (1) Facet Denervation Kit 100mm, part number 18PMK-21-100; one (1) Facet Denervation Kit 145mm, part number 18PMK-20-145; one (1) Facet Denervation Kit 50mm, part number 18PMK-21-50; one (1) Facet Denervation kit 100mm curved, part number 18PMK-20-100C; one (1) Facet Denervation Kit 145mm Curved, part number 18PMK-20-145C; one (1) case patient grounding pad (50 per case), part number 18PMA-GP-BAY and one (1) Extended warranty (1year). Functions and Capabilities: The self-calibrating Baylis PMG 115 generator is a computerized digital software platform enabling software upgrades which performs essential pain management procedures; standard radiofrequency, pulsed radiofrequency and intradiscal lesioning. It is capable of performing additional procedures with software upgrades. It is real-time digital and graphical display of critical treatment data (temperature, time, power and impedance) enables greater control of the procedure and safety. Intelligent probe recognition is enabled through the facet denervation kits (various sizes), which connect the generator to the patient through various sized cannulas. Quotations shall include all warranty information, technical data, etc. Quotations shall be submitted F.O.B. destination. All quotations are to include a completed copy of the provision at 52.212-3 Offer Representations and Certifications-Commercial Items, with quotation. (A copy of this provision may be requested by facsimile to (210) 617-5255, Attn: Joe Ortiz). The following provisions and clauses apply to this action: 52.212-4 Contract Terms and Conditions Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statutes and Executive Orders-Commercial Items including clauses incorporated by reference under Section b (11-16)(19)(21-22)(25), 52.212-1 Instruction to Offerors-Commercial Items, VAAR clause 852.211-77 Brand name or equal, 852.211-70 Requirements for Operating and Maintenance Manuals, 852.211-71 Guarantee Clause and 852.270-4 Commercial Advertising.
