Description
The VA Pittsburgh Healthcare System (VAPHS) is seeking a contractor to provide primary care services to veteran beneficiaries at contractor's Community-Based Outpatient Clinic (CBOC) located in Belmont County, Ohio. The Belmont County CBOC shall be solely dedicated to veterans. The area of consideration for the location of the facility is limitd to within the corporate city limits of St. Clairsville Ohio (County of Belmont) or no more than five (5) miles outside the corporate city limits of St. Clairsville, Ohio. The Goverment contemplates making a single award. The contract is for a Base Year plus four (4) Option Year periods. The solicitation allows a 60-day startup period. It is estimated that award will be made on or about July 3, 2003, and the first patient(s) will be seen at the CBOC on or about September 2, 2003. The contractor shall be qualified to provide primary care services, such as, but not limitd to: medical history, physical exam, clinical assessment diagnosis, treatment, Level I laboratory services as specified in the solicitation, ECG's, x-rays (excluding invasive procedures, MRI's, CT's, and ultrasounds), podiatry services, dietitian services, and Licensed Social Worker (LSW) mental health services. Laboratory specimens (other than the specified Level I laboratory tests) will be collected at the contractor's facility and forwarded to the VAPHS for performance. X-rays will be performed by the contractor and delivered to the VAPHS for interpretation. Physicians providing primary care services under this contract shall demonstrate evidence of education, training, and experience in Internal Medicine and/or Family Practice. The VAPHS prefers that physicians providing primary care services under this contract are certified in Internal Medicine and/or Family Practice by the American Board of Medical Specialties (ABMS) or the Bureau of Osteopathic Specialists (BOS). The enrollment at the current Belmont County CBOC as of March 31, 2003, was 4,936. For the Base Year, the Government estimates the workload to be 5,750 unique veteran patients with an individual veteran being seen an average of two (2) times. The offeror will quote a "per member per month" capitation rate and quote an hourly rate for the LSW mental health services for the Base Year and for each Option Year and will submit a written technical proposal addressing these evaluation factors: Past Performance, Staffing, Coordination and Continuity of Care, Site, and Small Disadvantaged Business Participation Program. Offerors are advised that they are responsible for obtaining amendments. Requests for hard copy solicitation must be in writing to Linda Glancy at the above address and accompanied by a non-refundable check for $5.00 made out to the VA Pittsburgh Healthcare System. No telephone requests will be accepted. A preproposal conference will be held at the VAPHS, University Drive Division, Pittsburgh, PA 15240; and the date, time, and conference room will be identified in the solicitation. The estimated proposal due date is close of business (4:30 P.M.) on June 10, 2003. This solicitation is unrestricted.
