- Description
- Program 1524-S, "Selective Service Machineable Forms." Term contract for the period beginning May 1, 2003 through April 30, 2004 with up to 4 option years through April 30, 2008. Specifications cover the production of five different machineable single-sheet forms requiring such operations as copy pickup, reproduction from government furnished camera copy, negatives, and/or digital files, typesetting of minor corrections, proofs, printed prior to production samples, printing in one, two, or four-color process including PERFORATING ON WEB OFFSET PRESSES WITH IN-LINE PERFORATING WHEELS, packing, and delivery. Quality level 2 with stated exceptions - Must function on stated equipment. Dimensions and perforation placement are critical. The predominant productin function is web offset printing with in-line perforating. Bidders who subcontract this operation will be declared not responsible. Approximately 10 orders per year with quantities ranging from approximately 50,000 to 1,500,000, with approximately 60% of the orders estimated to be between 300,000 and 800,000. The five different forms are: 12 x 8-1/2" (4/4), 8-1/2 x 11" (4/4), 12 x 8-1/2" (2/2), 8-1/2 x 11" (1/0), and 10 x 16" (4/4). Required stocks inlude 100 lb white tag and 24 lb. xerographic bond (see specifications for exact requirements). IMMEDIATELY FAX BID REQUEST ON FIRM'S LETTERHEAD TO 312.886.3163.
