- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F43LS330760500 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-13. This acquisition is a 100% small business set aside and is under North American Industry Classification System code 334519, with a small business size standard of 500. ITEM 0001 Quantity 1. VideoProbe XL Pro- Digital Model (Probe Only). Part #PXLD420A UNT (or equal). *Insertion Tube Diameter: 3.9mm outer diameter. *Insertion Tube Length: 2 Meter (6.7?) working length. *Digital Model XL Pro Feature Set (Probe Only): Handheld controller with 5 inch diagonal high resolution active matrix LCD, Soft XL Pro Case, Joystick-controlled motor driven All-Way tip articulation with Steer and Stay fine articulation, Home/ Return to Center function for probe orientation, Accessories ? Operators Guide, Integrated handpiece microphone, Freeze Frame and Image inversion, and Digital Zoom ? four step magnification. ITEM 0002 Quantity 1. Side View Tip for 3.9mm XL PRO VideoProbe. Part #PXT480SG (or equal). *80 Degree FOV, 4mm - 80mm DOF. The award will be based on the best value for the government. Please provide FOB destination pricing. Include shipping in the cost of each item not a separate line item. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR Clause 52.212-1, Instructions to Offerors- Commercial Items (Oct 2000). It is incorporated by reference and applies to this acquisition. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Ave. Bldg. 302, Altus AFB OK 73523-5132 at or before 1630 (4:30 P.M) CST, 28 April 2003. Submit dated quotations on company letterhead or quotation form. Quotations may be faxed to (580) 481-5138.
