Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The North American Industrial Classification System (NAICS) code applicable to this solicitation is 421450, Medical, Dental and Hospital Equipment and Supplies Wholesalers, and the small business size standard is 100 employees.=20 This is a full and open competition (unrestricted). The VA Healthcare Network Upstate New York at Syracuse New York intends to purchase a Medtronic Leadpoint 4 channel Micro Electrode Recording Unit, brand name or equal. Unit includes patient unit, lead point front-end pc card, and 33B39 earthcable, lead point software, switchbox (switches between 5 channels, 5 in and 1 out), notebook computer (Tecra 8200), Notebook PC (minimum specifications), including Windows 98, 850 Mhz Intel Pentium III Processor, 256 MB ram, 20 GB hard disk drive, internal CD R/W, 14.1 active matrix, TFT display (1024 x 768 internal & 1600 x 1200 external resolution), internal K56 modem and 10/100 ethernet line, 3.5 floppy disk drive, 16 bit sound blaster pro compatible, external mouse, external active speaker, micro targeting cable for lead point (10 each)(presterilized single use) sterilized 3-m active shielded cable for micro electrodes, DIN to 2 degrees 0.8mm and one alligator clip, including a clip for fixation of cable, black and white laser printer, knurr trolley for leadpoint. Included in the purchase is a one year comprehensive service agreement to include all software updates and upgrades, microtageting drive system with sterilization case, stepper motor drive (includes 1 box of drapes), Leksell stereotactic adapter, array electrode insertion tube set with carrier, microtargeting electrode 291A (bipolar 5 electrode array set, single use) length calibrated for Medtronic microtargeting drive system with adapter, and electrode sterilization tray. Salient characteristics of the system include (1) Simultaneous acquisition and display of 4 channels of microelectrode data (2) 1 to 5 simultaneous microelectrode recording tracks (3) Stepper motor drive with 1 micron resolution (4) electrode depth automatically displayed and saved with microelectrode data (5) automatic continuous saving or operator initiated saving of data (6) save to hard drive 10 ms to 30 second epochs of data (7) Faster display of saved epochs (8) Drive and microelectrodes are flash sterilized (9) Remote amplifiers connected to microelectrodes by single patient use cable supplied sterile (10) 50mm of drive travel available (11) Center guide hole on stereotactic axis with 4 holes offset by 2 mm on center and orthogonal to the center hole (12) Audio and visual playback of recorded data (=0913) On screen display of trigger frequency (14) Weight: amplifiers and notebook computer less than 13 lbs. (15) Amplifier Technical data (a) resolution: 16 bits (b) sampling rate: 14.0 kHz per amplifier (c) HW sample rate 3.072 MHz (d) filters: High pass 0.1 Hz =96 3 kHz (15 steps) Low pass 20 Hz =96 Open (12 steps) (e) common mode rejection: >100 dB (f) Isolation mode rejection: 16db. This solicitation document incorporates Federal Acquisition Regulation (FAR) provisions and clauses in effect through Federal Acquisition Circular 2001-12 and VA Acquisition Regulation=20 (VAAR) clauses as follows: 52-211-6 Brand Name or Equal; 52.212-1 Instructions to Offerors Commercial Items; 52.212-3 Offeror Representations and Certifications Commercial items; 52.212-4 Contract Terms and Conditions =96 Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items with (b)(11), (12), (13), (14), (15), (16), (18), (21) and (25) checked; and VAAR 852.270-4 Commercial Advertising. Offerors to include completed copy of 52.212-3 Offeror Representation and Certifications =96 commercial items. The provision at 52.212-2, Evaluation of Commercial Items, applies to this acquisition. The evaluation criteria stated in paragraph (a) of this provision are as follows: (1) Must be compatible with the Stryker Neuronavigation system model 6000 and the Leksell stereotatic headframe system Model 904120 and (2) price. The system offered must meet all the salient characteristics specified above to be considered for award. Award will be made to the offeror proposing an acceptable system at the lowest overall price.
