- Amended - - Questions are due May 12, 2003 to the Contracting officer. Answers will be posted withing seven (7) days. The proposal due date is revised to read June 18, 2003 for the following project. The Centers for Disease Control and Prevention contemplates awarding a contract for technical assistance for Assisted Reproductive Technology Data Collection. The purpose of this project is to collect annually, for the reporting years 2004, 2005, 2006, 2007, and 2008 clinic and cycle specific data (see Section P.4) from all practicing assisted reproductive technology (ART) clinics in the United States and its territories. Collected data will be used by the Centers for Disease Control and Prevention (CDC) to produce an annual report of pregnancy success rates for the public, as required in the Fertility Clinic Success Rate and Certification Act of 1992 [FCSRCA], Section 2 [a] of P.L. 102-493 [42 U.S.C. 263 (a) -1]. The period of performance is 84 months. Upon issuance, the solicitation will be available on the Internet at www.fedbizopps.gov. The date specified for receipt of offers will be approximately 30 days after the solicitation issue date. The solicitation issue date will be on or about April 7, 2003. Paper copies of the solicitation will not be made available. Telephonic requests for the solicitation will NOT be accepted. The NAICS Code is 541690. All responsible sources may submit a proposal which will be considered by the agency. It is each offeror?s responsibility to keep up with additional postings regarding this solicitation on the above referenced FedBizOps website. See Numbered Notes 25 and 26.
