Description
In accordance with FAR 12.603 (c) (2): This is a combined synopsis/solicitation for commercial items prepared IAW the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Stained Glass Mural is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-11. NAICS Code 23815 (Stained Glass Installation). Line Item 0001: The U. S. Air Force Academy in Colorado Springs has a requirement for a stained glass mural (window) in the Community Center Chapel. DESCRIPTION OF SERVICES: The contractor shall provide all labor, materials, supplies, and supervision to remove existing glass and install a stained glass mural encompassed in wood on the east wall of the Community Center Chapel, Building 5134. The contractor shall design a mural consisting of stained glass encased into wood framing. The design shall not have any denominational symbols associated with it. The design theme shall consist of uplifted arms or wings allowing sparkles of light to enter through window. The contractor shall install per approved design. Total work area is estimated to be 352 square feet. Contractor shall remove existing glass and wood framing and reinstall the new stained glass and wood frame per existing pattern. Thickness of glass to be used shall fit snug in wood channels. Contractor shall haul off all materials, glass, wood etc. being removed and not reused. Work shall be accomplished Monday through Friday 0730-1600 hours with no work during any type of church services or funerals. Contractor shall adhere to all Base Safety regulations during removal of existing and new installation of mural. Submittals of all equipment and designs to be used shall be approved prior to accomplishing. All work will be coordinated through the Contract Services Branch, Phil Martinez, at 719-333-3054. Location of Work: Work to be accomplished at the Community Center Chapel, Bldg. 5134 at USAFA. Time Restrictions: Work shall be held during normal duty hours, Monday ? Friday, 0730 ? 1600 hours. Services shall be performed in accordance with the best commercial practices. Offerors should include their DUNS, CAGE (Commercial and Government Entity) and Tax ID Numbers, and be CCR (Central Contractor Registration) and EFT (Electronic Funds Transfer) compliant. FAR 52.212-1, Instruction to Offerors-Commercial Items, applies to this acquisition. FAR 52.212-2, Evaluation-Commercial Items, applies to this acquisition, and the following factors shall be used to evaluate offers: Price and past performance, wherein price is considered approximately equal to past performance. FAR 52.212-3, Offerors Representations and Certifications, Commercial Items: and FAR 52.212-4, Contract Terms and Conditions, Commercial Items both apply to this acquisition with the following addenda to FAR 52.212-4, FAR 52.222-20, FAR 52.237-2 and FAR 52.242-15. FAR 52.212-5, Contract terms and Conditions Required to Implement Statutes or Execute Orders, Commercial Items applies to this acquisition. The following blocks are checked under FAR52.212-5, paragraph (b): Block (1) 52.203-6, with Alt I; Block (11) 52.222-21; Block (12) 52.222-26; Block (13) 52.222-35; Block (14) 52-222-36; Block (15) 52.222-37; Block (19), 52.225-3; Block (21) 52.225-13; and Block (24) 52.232-33. The following blocks are checked under FAR 52.212-5, DFARS 252.204-7004, Required Central Contractor Registration, and DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisition of Commercial Items. Offers must acquaint themselves with the regulation concerning Commercial Item acquisition contained at FAR 12.
