- Description
- This is a combined synopsis/solicitation for a commercial item prepar= ed in accordance with the format in FAR Subpart 12 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). The RFQ number is 246-02-00135 and should be referenced on all correspondence. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-12. The NAICS is 421450 and the size standard is 500 employees. Offeror=92s proposals are hereby requested in response to the requirements under a firm-fixed price contract and shall be submitted on offeror=92s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Quotes are to be provided for the following: Line item #1, one each electric powered operating table that meets the following minimum requirements: (a) must have two inch pads, (b) must have an x-ray top, (c) must have an auxiliary control and manual foot pump for back up in the event of power outage, (d) must have a 27=94 to 44=94 tabletop height adjustment, (e) must have side tilt and reverse orientation capable of supporting patients with a minimum weight of 500 pounds. Line item #2, one each universal leg-holder for the above table. Line item #3, one each drain pan for above unit. Line item #4 one each foot extension with locks for the above table. Line item #5, one accessory package that includes at least the following, anesthesia arm-boards with three inch pads, a perineal cutout filler piece, a cable wrap, and a conductive restraint strap. The quote is to include all shipping and any set-up charges and be valid for thirty days from the offer due date. All responses must include the following: (1) descriptive literature and product numbers for the items offered proving the products offered meet the above minimum requirements. (2) delivery time after receipt of order. (3) warranty information. The following provisions and clauses apply to this solicitation: 52.212-1 Instructions to Offeror-Commercial Items, 52.212-4 Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2 Service of Protest, 852.233-70 Protest Content, 852-233-71 Alternate Protest Procedure. Award will be made to the offeror whose offer meets the requirements listed above and is most advantageous to the Government. This is a best value procurement. The Government intends to evaluate the information received and award without discussions. Offers are to be submitted on company letterhead and include pricing information, the information required in provisions 52.212-1, a completed copy of the provisions of 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar or fax a request to 757-728-3132. Quotations and all required information must be submitted to Barbara Simmons, Contracting Office (90C), VA Medical Center, Hampton, Va. 23667 not later then 3:00pm local time April 24, 2003. Solicitation number RFQ 246-02-00135 shall be listed on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered. Electronic offers will not be accepted.
