Description

Provide temporary services of 1 part-time Registered Dental Hygienist (16 hours/week) for approximately 24 weeks. Services to be provided at the Coast Guard clinics located at: Ketchikan, Alaska. This will be a personal service contract. This requirement will be accomplished utilizing Far part 13, Simplified Acquisition Procedures. The contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours a day, 16 hours a week for a total of approximately 400 hours. Period of performance will be from 14 April 2003, or date of award through 30 September 2003. The Request for Proposals will be issued on or about 11 April 2003 and close on or about 30 April 2003. This procurement is a Small Business Set-Aside. The NAIC code for this acquisition is 621999, the SIC code is 8099 and the small business size standard is $5.0 million average annual receipts for the proceeding four fiscal years. All responsible sources may submit a bid, which shall be considered by the agency. The solicitation will be available through FEDBIZOPPS website at www.eps.gov. ATTENTION: Minority, Women-Owned and Disadvantaged Business Enterprises (DBEs) The Department of Transportation (DOT), Short Term Lending Program (STLP) offers working capital financing in the form of lines of credit to finance accounts receivable for transportation related contracts.
