- Oral Health expertise and capacity within State health departments are instrumental to coordination and collaboration among local health agency programs. CDC believes that resources devoted to oral health can be allocated more efficiently and effectively if they address locally defined priorities and, through statewide collaboration, are carefully designed to coordinate individual needs within a comprehensive plan for the community. These collaborations allow a broad spectrum of public and private educational, dental behavioral, social, not-for-profit, and health-care financing agencies to become involved with developing community-based solutions to the oral health problems that have been identified. To help State Oral Health programs identify successes and share lessons learned, it is important to assist them with the evaluation of their programs. This project is designed to build capacity within State Health Departments for evaluation. CDC expects to build on the evaluations of programs (coalitions, dental sealants, etc) in the future to assist other states with their oral health programs. Award of a firm-fixed price purchase order, using acquisition for commercial items/services procedures, is contemplated. The order will include the following periods of performance: Base Period---12 months from date of award; Option Period---12 months from date of successful completion of the base period and exercise of option to add this option period. Three additional options to include three additional states may be exercised at any time during the life of the project. This RFQ is a total Small Businesses set-aside. CDC will consider all responsible sources responding to the RFQ. All questions/concerns about the RFTQ shall be submitted via e-mail to BBiltz@cdc.gov. The tentative issue date is April 25, 2003. The complete solicitation package will be available and posted at this site for downloading when issued. No hard copy of the solicitation package will be mailed. The associated NAICS codes are 621210 and 54199 and the small business size standard is $6 million.
