Description
NA This modification is being issued to correct/replace the Statement of Work for Temporary Lodging Accomodations (APARTMENTS) for Battle Creek Air National Guard Base, Michigan. The correct Statement of Work should read as follows: Performance Work Statement (PWS) FOR APARTMENT LEASE, THE FOLLOWING TERMS AND CONDITIONS APPLY 1. LEASE CONTRACT TERM: The Basic Contract period for the Leasing begins on or about 1 May 2003. The government may be released from the lease agreement if the military orders of the individuals occupying the quarters are cancelled. In this event, the government shall provide the lessee with at least a 15 day written notice of cancellation. In the event the apartments are still required after the completion of the minimum quantities, the Government may continue to rent the apartments on a month to mont h basis for up through end of contract ordering period 15 February 2005. 2. SECURITY DEPOSIT: Is waived due to the government's self-insured status. The government will be liable for repairs and/or damages caused by negligence, carelessness, accident, or abuse caused within the time the government occupies the apartments. The government will be given the opportunity to inspect and review the repairs and/or damages prior to payment of such charges. Prior to occupancy, a government representative will jointly inventory and inspect the condition of the apartment(s). The apar tment manager will prepare a written record detailing the condition of the apartment(s) and any furnishings and appliances. A move out inspection will be conducted with a government representative. 3. FACILITIES. A. The contractors? property and units shall meet or exceed industry standards for cleanliness and d?cor. The contents of each apartment shall not be at the end of its life cycle, showing worn or shabby appearance. Each Apartment shall be full furnishe d, and as a minimum, shall consist of: 1. Bed, size-Full/Double, dresser/chest of drawers, desk and chair, 2. Bath and shower facilities, with hair dryers, 3. Fully equipped kitchen, to include range & oven, refrigerator, microwave, utensils, dishes, cooking ware, and coffee maker, 4. Central or individual heating and air-conditioning, 5. Color television, to include basic cable or satellite service, 6. Telephone and service for local calls only (Guests are responsible for their own long distance charges and will not be paid for under this contract.), 7. Iron and ironing board 8. All linens (bed, bath & kitchen) shall be provided B. Laundry facilities (washers & dryers) must be available, within the property, for occupants use. C. The contractor shall provide an adequate stock of cleaning supplies and equipment for occupants use. D. Apartment buildings shall have secured access. 4. UTILITIES: The following utilities will be included in this agreement and the cost of providing these utilities are included in the monthly rent charge. Water, Gas, Electricity, Basic Cable TV, Wastewater, Trash, Snow Removal, Lawn Service, Local Phone (No long distance). 5. The cost of providing the described apartments, including all utilities and miscellaneous services and supplies, shall be included in the monthly rent charge. 6. EXTRA CHARGES: The government will not be responsible for or pay for extras charges, such as long distance phone calls, additional upgrades or repairs, additional locks and latches, additional cable TV charges above and beyond what is provided in the mo nthly rental charge, pet deposits or pet monthly fees, and maid service. The lessee must collect any additional charges for these items directly from the individual(s) occupying the apartment(s). 7. Payment will be made in arrears of the rental month. Payment to be no later than 15 days after the receipt of invoice. The lessee must provide a valid invoice to the USPFO For MI, Attn: SGT Gay Mullen, 3111 W. St. Joseph Street, Lansing, MICHIGAN, 4 8913-5102, OR FAX TO 517-483-5900 . In order to allow for processing time, the lessee must provide an invoice not more than 7 days after end of rental month. 8. INDIVIDUAL LEASE CONTRACTS: As required by the lessee, the government individuals occupying the apartment(s) may be required to sign an individual lease contract to provide the individuals with the rules of personnel conduct required of the apartment c omplex. The individual lease contracts will not contain payment information. Payment information is contractual and contained in this contract between USPFO For Michigan and contract awardee. Additionally, each individual will be required to sign a stat ement required by the government to acknowledge that the individual is responsible for damage to the apartment when in their possession.
