Solicitation No. DTFA11-03-R-00041 Screening Information Request (SIR) for the Parking Lot Resurfacing and= Walkway Replacement Project, located at the Air Route Traffic Control Center, Salt Lake City, Utah Point of Contact: Michael Todd, Contracting Officer, (425) 227-1179. fa= x (425) 227-1055. The purpose of this Screening Information Request (SIR) is to seek competent and qualified contractors to perform parking lot resurfacing = and walkway replacement at the Salt Lake City Air Route Traffic Control Cen= ter facility. The Screening Information Request (SIR) is 100% set-aside for= Socially and Economically Disadvantaged Businesses (NAICS 23899). Socia= lly and Economically Disadvantaged Businesses (SEDB) competing on this acquisition shall be in good standing with the Small Business Administration. Certification documents are required for award consideration. Description of Work: I. Resurface and Reseal Parking Lot Provide all labor, materials and equipment to accomplish the following:= East and Southeast Parking Lots and drive between NNCC and ARTCC Buildi= ngs 1. Provide new asphalt overlay on approximately 95,000 square feet of= existing asphalt surfaced parking lot and driveway. 2. Re-strip parking stalls, traffic management indicators, walkways, = etc. West Parking Lot, Power Services Building, North and South ARTCC Drivew= ays 1. Slurry coat and crack seal approximately 125,000 square feet of parking lot 2. Re-strip parking stalls, traffic management indicators, walkways, = etc. Facility Exterior Walkway Replacement and Grounds Improvement Provide all labor, materials and equipment to accomplish the following inside the fenced area of the Air Route Traffic Control Center: 1. Remove and replace approximately 4350 square feet of sidewalk 2. Restore approximately 1300 Lineal Feet of sod next to new/replaced= sidewalk 3. Stripe and repaint sidewalk step noses with RED, non-slip paint 4. Place and finish approximately 600 square feet of new sidewalk 5. Ensure integrity of the existing irrigation system Provide all labor, materials and equipment to accomplish the following outside the fenced area of the Air Route Traffic Control Center (DESIGN/BUILD): It is the contractor's responsibility to verify all quantities. Standards and specifications used by Salt Lake City Corpora= tion can be found in APWA Utah Chapter Standards (www.apwa.utah.usu.edu) 700 North Street 1. Place approximately 780 linear Feet of new sidewalk 2. Lower existing manhole access to new sidewalk elevation 3. Form, Place and Finish approximately 120 LF of 18-inch high curb-w= all 4. Place approximately 2000 square feet of gravel, 3-inches deep betw= een new sidewalk and existing fence a. Scrape area under gravel to provide a level area, free from weeds,= etc. b. Install geo-mat under gravel c. Gravel can be NO lower than two (2) inches below bottom of fence 5. Remove all excess soil, debris, concrete etc. from the job site. 2200 West Street 1. Place approximately 762 Lineal Feet of new curb 2. Provide two drive approaches into the FAA property 3. Place approximately 740 Lineal Feet of new sidewalk 4. Place approximately 2000 square feet of gravel, 3-inches deep betw= een new sidewalk and existing fence a. Scrape area under gravel to provide a level area, free from weeds,= etc. b. Install geo-mat under gravel c. Gravel can be NO lower than two (2) inches below bottom of fence 5. Relocate four existing waste-water drains into the new curb line 6. Widen 2200 roadway from 700 North, approximately 775 feet northwar= d to the north FAA property line. Create a second northbound lane on 2200 we= st. (Widen roadway on 2200 West from existing asphalt pavement to the new c= urb line, approximately 27,135 square feet) a. Provide all traffic management as necessary i. Signs ii. Barricades iii. Flagmen if necessary b. Strip new roadway as required by Salt Lake City Corp 7. Provide all design and engineering data necessary to obtain a buil= ding permit from Salt Lake City Corp (building permit will be provided at no= cost to contractor) a. Site drawings b. Elevations c. Drainage plan d. Drain locations 8. Provide all necessary testing as required by Salt Lake City Corp a. Compaction b. Etc. 9. Remove all excess soil, debris, concrete etc. from the job site. East Frontage / East Fence Line 1. Furnish and place approximately 3000 Square Feet of =BE-inch minus= washed gravel on the outside of the fence line. (750 lineal feet by 4 f= eet wide gravel strip) a. Scrape area under gravel to provide a level area, free from weeds,= etc. b. Install geo-mat under gravel c. Gravel can be NO lower than two (2) inched below bottom of fence 2. Remove all excess soil, debris, concrete etc. from the job site. Estimated price range is $100,000 to $500,000. The estimated issuance d= ate for the Request for Offers (RFO) is March 19, 2003 with offers due Apri= l 2, 2003.
