- Amend the original description to read as follows: This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. Request for Proposal (RFP) 548-03-87 is advertised as full and open competition with NAIC: 621610 and a size standard of $11.5 million. The solicitation will issue on or about April 1, 2003. The VA Medical Center located at 7305 N. Military Trail West Palm Beach, Florida, seeks full and open competition for the award of a single or multiple award contract(s) for the delivery of home healthcare services to eligible veterans for a period of one year form the date of Notice to Proceed. This contract will include a (1) one year renewal option that can be exercised at the discretion of the Government. The total duration of this contract will not exceed two (2) years. The Government intends to award an indefinite delivery, indefinite quantity, firm fixed price contract(s) in accordance with the terms, conditions, provisions, specifications and schedule of the solicitation. Total number of awarded contracts will be determined based on the best value to the Govenrment, not exceeding ten (10) awarded contracts. The contract award will occur in the third quarter of Fiscal Year 2003, estimated to be toward the early part of June. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contract award may be based on initial offers reeived without discussions. Counties to be serviced by contract(s) are: Palm Beach, Martin, St. Lucie, Indian River, Okeechobee, Hendry and Glades County. Award(s) will be made so there are two (2) contractors per county. Contractor may submit a proposal for one county, all counties or any number of counties. The closing date for submission of proposal will be May 1, 2003 at 2:00 pm local time. No telephone or fax requests for proposal will be accepted. Any and all questions regarding the solicitation must be faxed to 561-882-6507 not later than fifteen (15) calendar days after the solicitation issue date.
