Description
A Commercial Solicitation is anticipated by the Alaska VA Healthcare System & Regional Office located at 2925 DeBarr Road, Anchorage, Alaska 99508. The services are going to be utilized in the Fairbank, Alaska area on an estimated amount of 24 addmissions annually. =20 SERVICES REQUIRED: The contractor will provide inpatient attending physician services for VA authorized inpatients at FMH as requested, and will be fully responsible for inpatient management from admission through discharge. The contractor shall be available for admissions from the VA Fairbanks Community Based Outpatient Clinic, as well as through the FMH Emergency Department. The contractor will facilitate the transfer of stable patients from FMH to Federal or other civilian facilities as necessary and appropriate. VA makes no guarantees of minimum volumes for services provided under this agreement. Utilization of this agreement will be entirely at the discretion of the VA. =20 All services shall be provided in a manner consistent with the patient=92s values and beliefs. The contractor shall obtain appropriate patient consent prior to rendering services. The contractor shall have a process in place for handling patient complaints. The contractor shall provide necessary patient education in a manner consistent with the patient=92s ability to understand. The contractor shall practice no discrimination based upon sex, race, color, national origin, creed, or religion. The contractor shall maintain records for all VA beneficiaries treated, and make copies of these records for inclusion into the patient=92s official VA medical record. The contractor shall maintain medical records for all VA beneficiaries treated in accordance with the Privacy Act of 1974 and housed in a physically secure area. All medical records shall be made available for the referring facility upon request for quality assurance activities. =20 The contractor shall be required to ensure a medical record is maintained for every patient treated under the auspices of this contract. Each medical record shall contain the following components as applicable to the service being provided: (1) appropriate patient identification to include name, sex, social security number, and date of birth;=20 (2) the patient=92s medical history; (3) evidence of consent for care when consent is required by organizational policy;=20 (4) documentation of care planning activities based on the patient=92s problems and needs; (5) all appropriate diagnostic and therapeutic procedures, treatments, and tests and their results;=20 (6) patient=92s response to care or services; (7) patient=92s vital signs, including assessment of pain; (8) findings of initial assessment and reassessment(s); (9) the description of any safety measures required to protect the patient from injury; (10) a description of the patients functional limitations related to the care or services provided; (11) specific and appropriate notes on the care or service provided; (12) a description of the patient=92s activity restrictions, if any, as related to the care or services provided; (13) a statement of any change in the patient=92s condition related to the care or services provided; (14) documentation of medication use and medication allergies or sensitivities; (15) legible and complete physician orders as appropriate and as required by law or regulation; (16) transfer forms, summaries, or copies of any records received from the transferring organization, documentation of patient and family education; (17) other individuals or organizations involved in the patient=92s care; (18) referrals to internal and external care providers and community agencies; and dietary restrictions. (19) Copies of all documents forwarded to the VA , such as discharge summaries will contain the vets name AND social security number. Discharge summaries shall contain the final diagnosis and list all operations and or procedures performed. The contractor shall provide subspecialty care within the expertise of its=92 personnel for VA authorized care. In the absence of the required expertise, the contractor shall arrange for subspecialty care. Health care provided shall be delivered in a manner consistent with the needs of the individual patient and within the community standards of care. Requests for companies to be included in this solicitation must be made in writing. Solicitation pack will be available on or around March 28, 2003 (fax is acceptable (907) 257-6711). Requests for bids will only be accepted from responsible businesses currently engaged in this type of commerce. Please reference Solicitation # FRQ-463-0027-03. NAICS Code 622110 Size Standard 29.0 Million with estimated project value being $250,000 to $500,000 over the course of the 1-year contract with four(4) one(1) year options to renew (5-years). A Firm Fixed Price contract is anticipated. Responses will be open on April 15, 2003.
