- Description
- The Department of Veterans Affairs (VA), VISN 10 has an ongoing progr= am to furnish artificial limbs and related services to those veterans that require them and are eligible for assistance. It is VA=92s intention to solicit proposals to small businesses and award multiple contracts on a competitive basis to preferred providers within each VISN 10- VA Medical Center=92s jurisdiction under the authority of Title 38 U.S.C. Section 8123. Facilities under the VISN 10-Jurisdiction are as follows: VA Medical Center, 17273 Street Route 104, Chillicothe, Ohio 45601; VA Medical Center, 3200 Vine Street, Cincinnati, Ohio 45520; VA Medical Center, 10701 East Blvd., Cleveland, Ohio 44106; VA Medical Center, 10000 Brecksville Road, Brecksville, Ohio 44141; VA Medical Center, 4100 W. Third Street, Dayton, Ohio 45428 and VA Medical Center, 2090 Kenny Road, Columbus, Ohio 43221. The NAICS is 339113 and the employee size standard is 500. Small Business entities that are interested in submitting L-code pricing for the above VISN 10 facilities must contact the Contracting Officer @ (440) 838-6091 or by fax (440)838-6052. The Government may elect to award a single delivery order contract or task order contract or award multiple delivery order contracts to two or more sources. The contract period shall be from May 1, 2003 through September 30, 2003 with four (4) one (1) year option renewals. The minimum dollar amount for supplies and services is $300.00 and maximum dollar amount for supplies and services is $300,000.00 per contract awarded.=20 =20
