- J=97TITLE: PREVENTIVE MAINTENANCE AND REPAIR OF HEMODIALYSIS EQUIPMEN= T SOL RFQ672-238-03 DUE March 10, 2003 3:30 PM America/Puerto Rico time. POC Rey Fco. Clemente, Contracting Officer (787) 641-2955 or (787) 641-7582 Ext. 10290 Facsimile number (787) 641-8364. Offeror shall provide all necessary labor, material and parts as required to maintain in fully operating condition hemodialysis equipment herein, located at San Juan VA Medical Center, (VAMC), San Juan, Puerto Rico for the period of April 01, 2003 through March 31, 2004 with option to renew for two additional twelve (12) months periods; which the Government may (or may not) exercise on a unilateral basis. Offferor shall provide a service technician the start of the day on the first working day of this contract to ensure all the equipment are in normal operating conditions; provide all inspections, (all inspections and preventive maintenance shall be done according to manufacturer=92s maintenance service manual instructions and procedures and these inspection shall be performed by a factory-trained and qualify personnel), provide adjustments, lubrications, installations of replacement parts and testing during the scheduled of Preventive Maintenance (PM) inspections; respond to requests for breakdown repairs =93same day=94 within four (4) hours of verbal request; Contractor shall respond to the service call within two (2) hours after notification; Contractor shall physically report on site VAMC within two (2) hours if the problem can not be corrected by phone; Contractor shall report directly to the Contracting Officer Technical Representative (C.O. T. R.) or his/her designee; Contractor shall supply all labor and parts, test instruments and tools required to perform all repair and maintenance work; the Service Technician shall submit each service report to the C.O.T.R or his/her designee at the conclusion of each service performance. Contractor shall submit a listing of parts that are not covered in this type of contract with their offer. Location of Contractor must be within a 50 miles radius of the San Juan VAMC. Offeror shall have and submit with their offer all applicable licenses and permits from the Federal Government, and Commonwealth of Puerto Rico. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A Firm Fixed Price Contract will be awarded.
