Description
This is a combined synopsis/solicitation for commercial items prepare= d in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation number RFP-797-FDF4-03-0007 is issued as a Request for Proposal. The solicitation document and incorporated provisions and clauses are those in effect through FAC 01-10. The NAICS code is 541611 and the small business size standard is $6.0M. This action is set aside for small business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for line item 0001 Non-Personal Services to perform a mock Joint Commission Accreditation of Healthcare Organizations (JACHO) survey for the 8th Medical Group, located at Kunsan AB, Republic of Korea, line item 0002 Non-Personal Services to perform a mock Joint Commission Accreditation of Healthcare Organizations (JACHO) survey for the 51st Medical Group, located at Osan, AB, Republic of Korea and line item 0003 Travel expenses in accordance with the federal travel regulation.=20 The successful offeror will be responsible for providing all labor, transportation, supervision, equipment, travel and expenses necessary to conduct a mock JACHO survey. Non-personal services will be for a minimum of 2 days at each location and include at least 2 surveyors for each location, one of which must be an administrator for Osan AB.=20 Service will include a pre-survey and executive meeting to discuss specific information requirements and evaluation procedures. A post survey executive brief, a comprehensive computer generated report scoring grid, action plan and an individual departmental critique of the process is required. At a minimum, a written and electronic copy shall be provided. Report shall contain at a minimum: identification of any type 1 deficiencies, deficiency sources and references for correction and training. Scoring grid format as per formal JACHO survey. Delivery is FOB destination to Kunsan AB during the week of 24 - 28 February 2003 and to Osan AB during the week of 3 - 7 March 2003.=20 Technical POC for both bases is Capt Michael D. Cupito at 011-82-31-661-6881, or michael.cupito@osan.af.mil. The following FAR clauses apply to this acquisition. FAR clause 52.212-1, Instructions to Offerors--Commercial Items; FAR Clause 52.212-2 Evaluation=97Commercial Items; FAR Clause 52.212-4, Contract Terms and Conditions--Commercial Items; FAR Clause 52.212-3, Offeror Representations and Certification--Commercial Items; and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification--Commercial Items, with the offer. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. This is a best value procurement. The following factors shall be used to evaluate offers: (1) PAST PERFORMANCE =96 Each offeror is responsible for having the attached questionnaire downloaded, completed and submitted to the Contracting Officer no later than the closing date and time of this solicitation. Three (3) questionnaires from three (3) different sources must be submitted; (2) PRICE =96 For evaluation purposes, the offeror=92s total evaluated price will be the total of line items 1, 2, and 3. Offeror is responsible for ensuring questionnaires are sent to the Contracting Officer at the address identified in this solicitation or faxed to (301) 619-3613, Attn: Ms. Wanda Gaines. Proposals shall be evaluated based on past performance that demonstrates the same or similar service as this requirement and price. Offers are due by 4:30pm EST on February 5, 2003 at the AFMLO/VASS, 1432 Sultan Drive, Suite C, Room 101, Fort Detrick, MD 21702-5006 (physical address) or AFMLO/VASS 1423 Sultan Drive, Suite 200, Fort Detrick, MD 21702-5006 (mailing address).
