Description
The Federal Bureau Of Prisons, FMC Bunter Laboratory intends to award an unrestricted firm-fixed price, requirements contract for the provision of analytes that will adapt to the BioMeriuex (MPP 3000) and that will perform infectious disease studies. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this requirement issued as a request for quotation is RFQ-10607-0019-3. The solicitation document incorporates the provisions and clauses in effect through Federal Acquisition Circular 2001-10. The applicable NAICS Code for this requirement is 423450 and a size standard of 100. The tests must fit a 96 well EIA format. The following are the BIO-RAD stock numbers, test names and their respective quantities required. The quantities are estimated and delivery orders will be issued on an as needed basis to ensure shelf life freshness. Item 0001: P001925 Hepatitis A Antibody IgM (96 Tests), Quantity 20; Item 0002: P001928 Hepatitis B Core Antibody IgM (96 Tests), Quantity 20; Item 0003: P001927 Hepatitis B Core Antibody total (192 Tests), Quantity 10; Item 0004: 32561 Hepatitis B Surface Antigen (480 Tests), Quantity 10; Item 0004: 32560 Hepatitis B Surface Antigen Confirmatory (10 Tests), Quantity 4; Item 0005: 930740 Hepatitis C Antibody (480 Tests), Quantity 16; Item 0006: 933404 HCV Stop Solution (each), Quantity 4; Item 0007: P001931 Anti Hepatitis B Surface (192 Tests), Quantity 15; Item 0008: 933730 HCV Wash (6 x 150 ml bottle), Quantity 4; Item 0009: 363 Tri-level Liqui-chek (4 per pack), Quantity 10; Item 0010: 685 Cardiac Tri-level (2 per pack), Quantity 10; Item 0010: 25030 Syphilis IGG (960 Tests), Quantity 20; Item 0011: 32508 Western Blot (40 Tests), Quantity 6. In addition, please provide any shipping and handling on the above mentioned items for deliveries made on an as needed basis. Delivery time frame for Task Orders issued will be two weeks after order is placed to vendor. The performance period for the Purchase Order will start the date of award through September 30, 2003. The acquisition incorporates the following JAR clause: 2811.104-70 BRAND-NAME OR EQUAL DESCRIPTION and the following FAR clauses: 52.211-6 BRAND NAME OR EQUAL (AG 99); 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (Oct 95) must be completed and submitted with your quote (please contact Arthur Johnson at 919-575-5000 ext. 1072 if a copy is needed); in accordance with FAR 52.212-5 EVALUATION-COMMERCIAL ITEMS-Evaluation factors, listed in order of importance, will be: Technical, Past Performance and Price. Technical and Past Performance, when combined are approximately equal to price. Offerors must submit information concerning three prior contracts regarding related supplies for the evaluation of past performance. A list of customer name, point of contact, telephone number and contract number. Bob Latina, Chief Laboratory Tech, has been appointed COTR(Contracting Officer?s Technical Representative) to assist with the evaluation of offerors proposed items.
