- Description
- The following changes have been incorporated into Solicitation Number F09607-03-00001: 1. The closing date has been extended to 31 Jan 2003. 2. Change the classification code from ?R ? Publication of Base Guide and Map? to ?T ? Guides, Maps and other Publications? 3. Replace the word ?contract? throughout the document with the word ?agreement?. 4. Replace paragraph 7. with the following paragraph: 7.Contract Terms: a. The agreement, unless sooner rescinded as herein provided, shall run for a period up to 2 years commencing on (award date). This agreement may be extended by mutual agreement on either a 1-year or 2-year basis for a total period not to exceed 6 years. The first issue for the Moody Air Force Base Guide and Map, and Moody Air Force Base Telephone Directory pursuant to this agreement shall be published in August 2003. b. This agreement may be rescinded by the contracting officer prior to its expiration whenever the products and services to be provided by the publisher hereunder fail to comply with the provisions hereof, and the publisher, after written notice from the contracting officer of such failure, fails to correct same within a reasonable time. Moody Air Force Base shall not be obligated to renew this contract. c. The contracting officer may also rescind this agreement for the convenience of the Government in the event the Government no longer requires this service. If your firm wishes to compete for the opportunity to publish these publications, please submit a written proposal and samples of your past publications, base guides and maps and/or telephone directories to: 347 CONS/MSCB, ATTN: Jacqueline Head, 4380B ALABAMA ROAD, MOODY AFB, GA 31699-1794. Please ensure your proposal addresses the requirements in the Instruction and Notices to Offerors and your ability to meet the selection criteria for the award evaluation. Publishers should ensure they sign their proposals on the last page of the Statement of Work, as the government may execute an agreement on the basis of the initial offers received without discussion. Please direct any questions concerning this Request for Proposal to Jacqueline Head at (229) 257-4704 or the contracting officer, Patericia Solberg, at (229) 257-4715. 8. Request contractor submit a listing of five (5) references, to include name of base/company, complete address and telephone/fax number, for whom the offeror has performed a similar agreement of services for Publication of Base Guides and Maps with the Federal, State and Local Government during the past two (2) years. For copy of questionaire call POC: J. Head. NOTE: Verification will be conducted in the form of telephonic questionnaires referencing past performance. 9. Incorporate the following paragraph: Selection Criteria: a. While the Government and the Contracting Officer will strive for maximum objectivity, the evaluation process, by its nature, is subjective and therefore, professional judgment is implicit throughout the entire evaluation process. b. Selection will be based on the contractor meeting technical requirements as mentioned in the sources sought requirements, past performance ratings and publication creativity. c. Past performance shall be evaluated and rated on the basis of the following subfactors each of equal importance as follows: (1) Quality (2) Management Effectiveness (3) Timely Performance d. A number of past performance references provided by offerors will be selected and investigated. Any derogatory past performance information obtained will be presented to the affected offeror and that offeror will have an opportunity to respond with any mitigating facts. This exchange of information will be considered to be clarifications and not discussions. The final determination of past performance ratings shall rest with the Contracting Officer. e. PAST PERFORMANCE: Past performance confidence assessment shall be accomplished for all offerors, and each offeror shall be assigned one of the following ratings: Exceptional/High Confidence: Based on the offeror?s performance record, essentially no doubt exists that the successful offeror will successfully perform the required effort. Very Good/Significant Confidence: Based on the offeror?s performance record, little doubt exists that the offeror will successfully perform the required effort. Satisfactory/Confidence: Based on the offeror?s performance record, some doubt exists that the offeror will successfully perform the required effort. Neutral/Unknown Confidence: No performance record identifiable, or applicable (see FAR 15.305(a)(2)(iii) and (iv)). Marginal/Little Confidence: Based on the offeror?s performance record, substantial doubt exists that the offeror will successfully perform the required effort. Changes to the offeror?s existing processes may be necessary in order to achieve contract requirements. Unsatisfactory/No Confidence: Based on the offeror?s performance record, extreme doubt exists that the offeror will successfully perform the required effort. f. An offeror with limited or no recent or relevant past performance will receive a rating of Neutral/Unknown Confidence, and will not be rated favorably or unfavorably on past performance.
