- The Internal Revenue Service intends to issue an order to Eastman Kodak Company, 343 State Street, Building 20, Rochester, NY 14650-1177. This contract action is for Annual Maintenance for 12 EK/1000 FB Flatbed Scanners SA (6) Ogden,Utah S/N: 8012224,8012211,7082006,12098,00052096,00122047 (1) Covington, KY S/N: 00122102 (1) Kansas, MO S/N: 00122042 (3) Ogden, Utah S/N: G8007052132, G8008092103, G8008022221 (1) Austin, TX S/N: 00122092 3 EK/8125 Copiscan SA Ogden, Utah S/N: MYG3720031, NZF5570371, MYR5570324 Government intends to solicit and negotiate with only one source. Interested parties may identify their interest and capability to respond to the requirement by submission of a proposal. Vendor signed proposal must include price, terms and conditions, point of contact (name, telephone number, e-mail address and fax number), Government contract or BPA number (if applicable), performance period, business size, payment terms, taxpayer identification nubmer (TIN) and Dun and Bradstreet number. This notice of intent is not a request for competitive proposals; however, all responses must be received by 3:00 p.m., January 21, 2003. All responses should reference number M-3-M0-24-CS-A11. If no proposal is received that provides clear anad convincing data in writing substantiating on ability to perform the entire requirement, award will be made to Eastman Kodak Company.
