- Description
- The Northern Arizona VA Health Care System, Prescott, Arizona is seeking to contract for a variety of home infusion therapies. Services are to be provided to eligible veterans residing in Yavapai, Navajo, Mohave, La Paz, Apache or Coconino counties in the State of Arizona. Therapy includes: pharmaceuticals, equipment, hazardous waste disposal, pharmaceutical monitoring, consultation and management. Pharmaceutical reimbursement is at or based on the Average Wholesale Price (AWP) with services based on a per diem rate. The Government contemplates a firm fixed price commercial items requirements contract for a base period and three (3) renewable option years. Providers must be licensed by the State of Arizona and JCAHO accredited. All interested parties may view the solicitation on this website. A copy is anticipated to be posted not later than 12/17/02. The NAICS Code 621610. Size Standard $11.5 Million.
