- Description
- This is a solicitation for commercial items prepared in accordance wi= th the format in FAR subpart 12.6 as supplemented with additional information included in this notice. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-10. RFP 658-16-03 should be referenced on all proposals. The applicable NAICS code is 621498 with a size standard of $8.5 million. This acquisition is unrestricted. The VA Medical Center, 1970 Roanoke Blvd., Salem, Virginia 24153 has a requirement for a Community-Based Outpatient Clinic (CBOC). The VA Medical Center, Salem, Virginia has a requirement to utilize a pre-existing clinic in the Danville, Virginia area, complete with primary care physicians and personnel required for primary care and routine primary care services. At the time of response to this solicitation, the clinic must be in compliance with JCAHO requirements. The clinic must have either on-site lab and radiology capability OR be able to demonstrate that a contractual relationship exists to provide off-site lab and x-ray services to the existing clinic. Clinic must be able to cover the following geographic locations: counties of Henry, Patrick, Pittsylvania, cities of Martinsville, Danville and Laurel Fork, Virginia and be located within the VA Medical Center, Salem=92s primary service area. Physicians, nursing staff and technical staff providing services under the resultant contract must meet all evaluation criteria and other professional certifications and licensing required. The qualifications of such personnel shall also be subject to review and approval by the Chief of Staff of this Medical Center.=20 The VA Medical Center, Salem, Virginia intends to negotiate a capitated rate per patient. There are approximately 21,138 veterans residing in this service area; however, it is anticipated 2943 of those 21,138 veterans will utilize the clinic in the first year. Clinic profiles will be established as clinic staff deems appropriate to accommodate workload demand. This contract is for the period of April 17, 2003 through April 16, 2004 with four (4) available option year renewals.=20 The solicitation will be available on or after December 31, 2002. A pre-proposal conference has been tentatively scheduled for January 22, 2003. Definitive information on the conference will be contained in the solicitation. Copies of the solicitation may be obtained at no charge at the Business Opportunities System website http://www.bos.oamm.va.gov. Solicitation dissemination is limited to electronic medium and no hard copies will be provided by the VA. Any amendments will be posted at the website. Offerors are advised that they are responsible for obtaining all documents and amendments.=20 Offers submitted must have an original signature by an individual authorized to bind the company. Electronic offers will not be accepted. All responsive, responsible sources who submit a proposal that is received by 4:30 p.m. EST, March 3, 2003, will be considered by the Government.
