- Description
- Contractor shall provide Laser Surgery Services at the VA Medical Center, 10535 Hospital Way, Mather, California 95655-1200. Contractor shall assign qualified Laser Technicians to delivery equipment and assist VA surgeons in the effective application of lasers, and other high technology devices, during surgical procedures. This will be a unrestrictive commercial procurement for a base and two one-year options. The following clauses and provisions plus any addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluations, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions, and 52.212-5 Terms and Conditions to Implement Statutes and Executive Order. The solicitation will be available o/a 12/10/03 at http://www.bos.oamm.va.gov/cgibin/WebObjects/BosPublic.woa No email, fax or telephone request for solicitation documents will be accepted. All questions must be in writing and may be emailed to John.Gregorio2@med.va.gov or faxed to (415) 750-2170. No web based offers will be accepted. The offeror is responsible for downloading all documents and amendments without further notice by the VA.
