- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A quote is being requested and a written solicitation will not be issued. Solicitation number F73COM22130400 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. This acquisition is unrestricted and is under North American Industry Classification System Code 517211. The contractor shall lease and provide service for 365 Pagers at Altus Air Force Base of those, the contractor shall provide six each group calls for the pagers. The group calls are as follows: Group One ? 97 Medical Group-23 Pagers, Group Two ? 97 Medical Group-5 Pagers, Group Three ? 97 Civil Engineering Squadron-10 Pagers, Group Four ? 97 Civil Engineering Squadron / Fire Department-40 Pagers, Group Five ? Disaster Control Group-19 Pagers, Group Six ? Crisis Action Team-6 Pagers, All pagers shall be programmed for one zone that covers all of South Western Oklahoma to include Oklahoma City, Tulsa, Stillwater, Eldorado, Clinton and Wichita Falls Texas. The Contractor shall provide all maintenance on the pagers at no cost, unless damage is caused by blatant misuse on the part of the Government. The access number for the pager system shall be a local number for Altus. If for any reason there is going to be an interruption in service the Land Mobile Radio (LMR) office at Altus AFB shall be contacted 24 hours prior to cut off. Contractor shall submit their pricing on the attached Pricing Schedule, which is available for download. The contractor shall meet the requirements of the Statement of Work, which is available for download. Period of performance is for a base year plus 4 option years. The basic year performance period is 01 Jan 2003 through 31 Dec 2004. Contractor shall be able to accomplish any requirements to enable continued service as of 01 Jan 2003. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR clause 52.212-1, Instructions to Offerors-Commercial Items (Oct 2000). It is incorporated by reference and applies to this acquisition. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Avenue, Bldg 302, Altus AFB OK 73523 at or before 1630 (4:30 pm) CST, 13 Dec 02. Submit signed and dated quotations on company letter head or quotation form. Quotations may be faxed to (580) 481-5138. Offers on this acquisition shall be evaluated on the basis of price factors only. FAR Clause 52.212-2 Evaluation ? Commercial Items (Jan 1999), FAR 52.212-2 (a) is amended to read: (i) price. The government reserves the right to make award on the initial quote without discussions. Each offeror shall include a completed copy of the provisions at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Item (July 2002) with the quotation. If a copy of the provisions are needed, please request one from the point of contact, and a copy will be faxed. FAR clause 52.212-4 (Feb 2002), Contract Terms and Conditions-Commercial Items, is incorporated by reference and applies to this acquisition with the following addendum: DFARS 252.204-7004 Required Central Contractor Registration (Nov 2001). FAR 52.212-4(c) is tailored as follows: (c) Changes. Changes in the terms and conditions of this contract may be made only by written agreement of the parties with the exception of administrative changes (such as paying office, appropriation data, etc), which may be changed unilaterally by the Government. The clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (May 2002) DEVIATION applies to this acquisition and is incorporated by reference. A Firm-Fixed Price purchase order will be issued in writing. No telephonic responses will be processed.
