- Description
- This procurement is for comprehesive annual eye exams for eligible veteran beneficiaries of the Department of Veterans Affairs Medical Center, Grand Junction, CO. Eye exams are to be performed by a Board Certified Optometrist who possesses a full and unrestricted license to practice in the State where services are performed. This is for the contract period January 1, 2003 through December 31, 2003 with 4 option years. This will be an IDIQ (Indefinite Delivery Indefinite Quantity) Firm Fixed unit price contract. Estimated number of exams per year is 950. Pas performance /demonstrated capability, quality assurance and personnel qualifications are of equal importance and are significantly more important than cost/price. Contract award is subject to availability of funding. This procurement is 100% set-aside for small business. The NAICS code is 621320 and the size standard is $6.0 million in average annual receipts for the last three fiscal years.
