Description
Provide professional engineering services to Conduct a Statement of Conditions Survey at each of the VISN 12 facilities. A Statement of Conditions Survey will be conducted at each VISN 12 facility. The purpose of the Statement of Conditions survey is to identify and address Life Safety Code deficiencies that must be resolved so that compliance with JCAHO standards is achieved and maintained. The award firm will use the 2003 JCAHO Standards published in the Comprehensive Accreditation Manual for Hospital: the Official Handbook (CAMH) and follow JCAHO guidelines regarding the Statement of Conditions. The survey process will focus on the 2002 edition of the Life Safety Code and will cover only those buildings where a Statement of Condtions is required by JCAHO. All Surveys will be completed no later than 90 days after contract award and a written report, including CAD drawings, will be required within twenty (20) business days after each site visit. Follow up technical support will be provied by telephone/electronic mail to all VISN 12 facilities on Statement of Condition issues leading up to the JCAHO 2003 surveys. Dates of the survey may be negotiated with each facility. After all surveys are complete, the surveyor will schedule an exit conference with the VISN 12 Network Director and other leadership staff to discuss findings and trends. The Consultant shall possess the appropriate qualifications and credentials for conducting the surveys. He/She will be a licensed professional engineer with specific health care experience. Submit two (2) copies of SF 254 and SF 255 with appropriate documentation by close of business December 20, 2002. Selection criteria will be based on: (1) Knowledge and experience with JCAHO and Life Safety Code Standards, (2) Past performance with similar projects, (2) Qualifications of staff to be assigned, with special emphasis on field personnel to be utilized, (3) Ability to meet specified timeframes, No bid packages are available. This is not a Request For Proposal (RFP). NAIC #541310 applies. Award is subject to the availability of funds.
