Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, supplemented by FAR Part 15 with additional information elsewhere in this notice. This announcement constitutes the only solicitation, proposals are requested and a written solicitation will not be issued. The Government intends to evaluate proposals and make award without discussions. The period of contract performance will be from date of award, approximately 1/1/03 through 9/30/03, with four one-year renewal options. Award is based on availability of funds. The VA is currently operating under a Continuing Resolution through 1/11/03. If another continuing resolution is signed, or the budget is signed, proper documentation will be provided the contractor for signatur Solicitation number 659-28-03 is assigned this procurement. The solicitation document an dincorporated provisions and clauses are those in effect through FAC 2001-9. This is a competitive procurement under the NAICS code 422210 and open to both small and large businesses. The contractor shall furnish and deliver radiopharmaceutical supplies as listed herein, FOB Destination to Room 2168, Building 2 at the Hefner VA Medical Center, 1601 Brenner Ave., Salisbury, NC 28144. Orders will be requested by authorized staff in accordance with Public Law 104-262 and 38 USC 8153, on an as-needed basis and may be placed throughout the day. Individual radiopharmaceuticals will be provided in unit dose form. Deliveries will be made the following workday morning no later than 7am. These desliveries are daily, Monday through Friday, to except Government holiday. The ten Federal holidays are New Year's Day, Dr. Martin Luther King Jr. Birthday, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran's Day, Thanksgiving Day, and Christmas Day. The Contractor must be licensed by the Nuclear Regulatory Commission and be regularly established in the providing radiopharmaceuticals. A copy of the offeror's license and certifications must be provided with the proposal. A single award will be made from this solicitation. Offers should provide procing for the base year and each option of the 4 one-year renewal options. The Department of Veterans Affairs has implemented a Department-wide standardization program. As a result, VA is issuing National Consolidated Contract(s), BPAs, and BOAs for standardized items. When items in this agreement are standardized under any National Consolidated Contract, BPA, or BOA, they will be deleted from this agreement 60 days after notice from the Contracting Officer. The quantities stated herein are estimated and based on past history. There are 17 CLIN (Commerical line item numbers). The offeror shall provide pricing on the offeror's letterhead for the base year and each option year. The offeror shall include a statement in his/her letter that the offeror accepts the conditions and terms as stated in the solicitation. The CLINS are: (1) 99mTcHDP, 20mCi/dose; est. qty 144 dose; (2) 99mTcDTPA, 15mCi/dose; est qty 12 dose; (3)99mTcDTPA, 35mCi. est. qty 6 dose; (4) 67 Gallium, 6mCi, est. qty 4; (5)66mTcDSF, 5mCi, est. qty 14 dose; (6)111 Indium WBC Tagging .500mCi/dose, est. qty 2 dose; (7)99mTcSc, 5mCi/dose, est. qty 24 dose; (8)99mTcMAA, 3Mci/dose, est. qty 2; (9) 99mTcMAA, 4.5 mCi/dose, est. qty 6 dose; (10)99MTc Pertechnetate, 10 mCi/dose, est. qty 12 dose; (11) 99mTc Pertechnetate 25mCi/dose, est. qty 350 dose; (13) Aerosol Ventiliation Kit (Ultravent)(Mallinckrodt) est. qty 6 kits; (14)99mTcPYP, 20mCi/dose; est. qty 2 dose; (15) 131 I 5-15 uli, est. qty 8 dose; (16) 99mT-Cardidite, 25 ml, est qty 192 dose and (17) Lincarity, est qty 4 dose. Contractor will be responsible for pick-up and disposal of all radioactive waste generated by the use of the Contractor's radiopharmaceuticals. This will include needles, syringes and any contaminated items such as absorbent pads, paper towels, pens, etc. Contractor will submit invoices monthly, in arrears, to Hefner VAMC, Attn: Ozzie Marin (114), 1601 Brenner Ave., Salisbury, NC. Invoices shall include an itemized list of all procedures performed during the month, listed by prescription number, patient name, procedures, date and time. Offeror shall provide a credit percentage if allowed for any returned doses. Contractor shall provide company name, address, contact person and telephone number where service calls are to be placed. Award will be made bsased on price, past history and technical performance with all factors being considered equal. Questions may be directed to the Contracting Officer at jolette.cole@med.va.gov.
