- The Centers for Disease Control and Prevention intends to purchase Professional Consultant and Operational Services from Charlotte Dickinson of Norcross, GA. The Government intends to procure technical assistance, consultation and operational support related to program planning, evaluation and assistance with the following: 1) Educate general public, health care professionals, and policy makers on issues related to the National Center on Birth Defects and Developmental Disability (Center); 2) Provide advice, participate in the development of, and prepare formal documents on programmatic vision, goals, and plans; and, 3) Help determine how well Center programs are achieving the desired goals and document the results of this evaluation. The Government intends to evaluate offers and award a contract without discussions with Offerors providing the requested services. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. The North American Industry Classification System (NAICS) code is 541611 and the Size Standard is $6 Million for this requirement. This requirement is a SMALL BUSINESS SET ASIDE. Interested vendors capable of furnishing the Government with the services in this synopsis should submit two (2) copies of a quotation to the below address. The quotation shall include the itemized price, prompt payment terms, the F.O.B. Point (Destination or Origin), Taxpayer Identification Number (TNN), Dun & Bradstreet Number (DUNs), and descriptive literature of the product quoted. The quotation must reference Solicitation number NCBD11-03. Quotations will be due fifteen (15) calendar days from the publication date of this synopsis or 12/4/02, by 8:30 a.m. local time, whichever is later. All responsible sources may submit a quotation which, if timely received, shall be considered by the agency. Quotations must be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Lenore Mickle. Email quotations will only be accepted if dated and signed by an authorized company representative to: Lmickle@cdc.gov. Charlotte Dickinson is the only source known to the Government who can provide a service with the above characteristics and possesses knowledge of the Centers planning and evaluation programs. However, if any interested party believes it can meet the above requirements, it may submit a statement of capabilities. The capability statement and any other furnished information must be in writing and must contain material in sufficient detail to allow CDC to determine if the party can fully meet the requirements herein. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR Part 12.102(c) under the test program. A determination by the Government not to compete this proposed requirement based on responses to this notice is solely within the discretion of the Government. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). OFFERORS SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERORS REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH YOUR QUOTE. This procurement is not set-aside for small business.
