Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation/synopsis is being issued as RFQ 60605-0001-3. This solicitation/synopsis and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-09. Note #1 of Numbered Notes applies. NAICS code 621999. This is a requirement for a Pharmacy Technician services to inmates housed at the Metropolitan Detention Center, Los Angeles, CA(MDCLA). QUALIFICATIONS: The provider shall hold a minimum of a high school diploma and have either a certificate of appropriate training for Pharmacy Technician services and/or one year minimum experience as a Pharmacy Technician. All claims of education, training and experience must be verifiable. All duties will be performed in accordance with standards and methods generally accepted within the provider's field of expertise, including but not limited to the following STATEMENT OF WORK: 1)Provides clerical and administrative support in the pharmacy, which includes access to: order processing, stocking, monitoring expiration dates, pharmacy record management, distribution of pharmaceutical supplies. 2)Interprets physicians' and physician assistants' prescription orders; entering the prescription orders into the pharmacy computer and printing prescription labels; selecting the correct medication and preparing the prescription for the pharmacist to verify; maintain inventory of medications and supplies; entering data for medications and supplies in the prime vendor computer system; dispensing, administering and recording entries for the required medications. 3) Serves as a resource person to the Health Services staff in regards to routine inquiries concerning Pharmacy policies and procedures, patient profile information and availability of pharmacy supplies. 4) Submits statistical reports, documentation of work performed and familiarization with administrative guidelines of the JCAHO, DEA, American Correctional Association (ACA), and the BOP Health Services Program Statement. 5) Participates in the organizational Performance Improvement Program. 6) Must have knowledge of sterile technique and be able to ascertain atypical occurrences such as change in color or consistency and must be able to perform pharmaceutical calculations, such as metric conversions and dosage adjustments. 7) Inspects drugs for adequacy, overstocking, arrangement storage proper rotation and deterioration and maintains a perpetual inventory of all drugs and assumes responsibility for security of drug sin his custody and ensures compliance with DEA requirements. 8) Uses proper techniques for storage of drugs, such as refrigeration or protection from heat and light. 9) Must be alert to avoid any possible changes which may result in deterioration of the product and loss of its potency, formation of toxic products, or other undesirable effects in addition to adverse patient care. Must be able to recognize unusual occurrence and report them to the pharmacist. 10) Must refer all adverse drug reactions, drug interactions, and unusual problems or requests to the Pharmacist or Contract Monitor /COTR. 11) Reads individual prescriptions and determines kind, strength and dosage dispenses. Correlations of instructions on the order and Pharmacy policy stations must be known. 12) Sets up prescriptions manually or via Pharmacy computer. Patient profiles are reviewed for change in therapy, allergies, drug interactions and timeliness of renewals. 13) Dispenses medications during scheduled hours as required by institution policy. Receives and fills medication orders from Sick call and Outpatient Chronic Care Clinic. Note: Contractor may dispense controlled substances but will not have the responsibility of the bulk storage of DEA substances stored in the main vault. 14) Contractor may receive pharmaceutical supplies into stock from the Warehouse. 15) Coverage in case of absence must be maintained by providing a qualified alternate pharmacy technician. The COTR must be notified of the changes by calling (213) 485-0439 extension 114 or 365. 16) During the contract period, contractor shall furnish current copies of all certifications, any continuing education received, proof of professional, medical liability insurance, and a copy of diploma(s) for credentialing pursuant to JCAHO standards and Bureau of Prisons policies, upon the request of the Contracting Officer or the Contracting Officer's Technical Representative (COTR). 17) SCHEDULE: Hours may be flexible and scheduled by the COTR. Under normal circumstances, services are required for six (one-hour) sessions per day, not to exceed 30 sessions (30 hours) a week, normally from 10:30 a.m. to 5:00 p.m. (unpaid half hour lunch), but is subject to change as required by COTR. All hours worked must be documented on a "Log of Consultants Visit" form within the HSU with each entry being completed and signed by the contractor. Task orders may be issued in writing, orally, by facsimile, or by electronic commerce methods. 18) All equipment, supplies, and materials that the contractor may routinely need to perform the services will be provided by the Government. Contractor will maintain accountability for government furnished equipment/supplies/materials. All medical records remain the property of the BOP. 19) This will be a contractual arrangement and not a personnel appointment; payment is based on the provision of an end product or the accomplishment of a specific result. The contractor will not be subject to Government supervision, except for security related matters. However, contractor performance will be closely monitored. 20) Payments under the contract will not have provisions for state, municipal, or federal withholding taxes, nor will payments be subject to the Federal Insurance Contributions Act. It will be the contractor's responsibility to report all earnings to the appropriate revenue service for tax purposes. (Note: Federal agencies are required to report all payments to a contractor to the Internal Revenue Service.) The contractor is not entitled to unemployment compensation benefits under the Social Security Act, as amended, by virtue of performance under the contract. The contractor agrees to comply with the Privacy Act of 1974 and the rules and regulations issued pursuant to the Act in the performance of the services required by the contract. FAR 52.237-07 INDEMNIFICATION AND MEDICAL LIABILITY INSURANCE, applies to this contract. The Contractor shall maintain during the term of this contract liability insurance issued by a responsible insurance carrier of not less than the following amounts(s) per specialty per occurrence: $1,000,000.00. SECURITY ISSUES: Any individual who provides service to the BOP must submit to, at a minimum, the following security investigations: National Crime Information Center (NCIC) check, fingerprint check, name check, employment history check. The Base Period will be from date of award (approx. 11/01/02) through 09/30/2003, plus 2 two option years, 10/01/03 to 09/30/04 and 10/01/04 to 09/30/05. FAR Provision at 52.212-1, Instruction to Offerors Commercial Items, applies to this acquisition and hereby incorporated by reference. FAR Clause 52.212-2 applies and is incorporated by reference. The Government intends to make a single award to the offeror offering the best value to the Government, price and other factors considered. The other factors include Technical Proposal, Experience and Past Performance. A copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items must be included with offers. FAR Clause at 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. FAR Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition.
