Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, N00014-03-R-0001, is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09 and DFARS Change Notice 20020730. The associated North American Industry Classification System (NAICS) code is 713940 and small business size standard is $6.0 Million. The Office of Naval Research (ONR) has a requirement to provide fitness services through a Corporate Membership Program to ONR civilian and military personnel. Services are to be rendered from date of award through one-year, with four one-year options. Offeror must provide health club facilities in the Ballston Metro area, which are within ten minutes walking distance from ONR?s offices (800 N. Quincy Street, Arlington, VA). Facilities should provide for the following: Resistive weight training machines, Cardio-fitness equipment, Aerobics classes, Free-weights & related apparatus, and sanitary locker rooms. ONR?s Corporate Membership Program provides ONR personnel a monthly subsidy for health club membership dues. That portion of monthly dues not covered by ONR?s subsidy will be the responsibility of the each individual ONR employee enrolled in the program and the offeror must allow for payment directly by each employee. The offeror must also provide for the following under this effort: a health risk assessment for each new member, a monthly report to ONR listing current members and the amount of monthly utilization by each member, and liability insurance for all facilities used by ONR employees. The offerors should submit with their proposal the following: (1) Technical proposal; (2) Cost proposal; (3) Completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (JUL 2002), and Alternate I (APR 2002), as well as DFARS 252.212-7000, Offeror Representations and Certifications--Commercial Items (NOV 1995), which provisions can be found at the website http://farsite.hill.af.mil/; and (4) An affirmation that it has an active registration on the Central Contractor Registration (CCR), which is at the website www.ccr.gov. The technical proposal should include the following: (1) a full description of services, equipment, facilities, and any other benefits provided to health club members; (2) address of facility or facilities available to ONR personnel; (3) hours of operation for each location; (4) copy of certificate of liability insurance for all facilities available to ONR personnel; (5) description of polices and health club membership options available to employees leaving ONR; (6) acknowledgement and agreement of the reporting and health assessment requirements mentioned above; and (7) description of policies for direct payment by ONR personnel of non-subsidized portion of monthly dues. The cost proposal should include the following: (1) Separate section for each year (base and four option years) with the total monthly rate (including subsidy and employee funded amounts) for the following numbers of ONR personnel: 1-25, 25-50, 50-75, 75-100, and 100-150; (2) amount (if applicable) of any administrative fees assessed for new members; and (3) description of any other fees associated with health club membership, which can include fees of additional services not normally offered in standard health club membership. The Government reserves the right to make multiple awards as a result of this solicitation if it is in the best interest of the Government. The Government will award a firm-fixed-price purchase order(s) resulting from this solicitation to the responsible offeror(s) whose proposal conforming to this solicitation will be the most advantageous to the Government, price and other factors considered. The following will be used as evaluation criteria for this procurement: 1) Amount and range of services and equipment offered (must at least have minimum requirements specified above, but higher ranking will be given to larger number and quality of services and equipment offered); 2) Convenience of location to ONR offices, extent of hours of operation, and ability of ONR personnel to have access to additional locations outside Ballston Metro area; 3) Observed usage rates of facilities during peak hours; and 4) Price. The three technical factors, i.e., numbers 1 through 3 above, are of equal importance to each other. Price is of equal importance to the technical factors. The exact amount of ONR?s subsidy per member and amount of members will be determined after receipt of proposals for the following year. Monthly rates for each range of membership proposed will be included in the order for each option period. Each awarded option will allow for adjustment of total ONR personnel covered for that period. Delivery and acceptance of reports is F.O.B. Destination to Office of Naval Research, Ballston Tower One, 800 North Quincy Street, Arlington, VA 22217-5660.
