- Description
- Provide Registered Dental Hygiene services at the Coast Guard Clinic located at Seattle, Washington to beneficiaries referred for treatment by a Coast Guard Dental Officer. This will be a personal service contract. This requirement will be accomplished utilizing Far Part 13, Simplified Acquisition Procedures. The work requires two contract providers? weekdays at different days and hours as established by the Clinic Supervisor. Contract Provider 1. will not exceed 9 hours a day 27 hours a week for a total of 486 hours per year. Contract Provider 2 will not exceed 8 hours a day 16 hours a week for a total of 288 hours per year. Period of performance will be date of award through January 31, 2003. The Request for Quote will be issued on or about October 3, 2002 and close on or about October 15, 2002. This procurement is a Small business Set-Aside. The NAIC code for this acquisition is 621399 and the small business size standard is $5.0 million average annual receipts for the proceeding three fiscal years. All responsible sources may submit a bid, which shall be considered by the agency. NOTE: The solicitation will be available through FEDBIZOOPS website at www.eps.gov. ATTENTION: Minority, Women-owned and Disadvantage Business Enterprises (DBEs). The Department of Transportation (DOT), Short Term Lending Program (STLP) offers working capital financing in the form of lines of credit to finance accounts receivable for transportation related contracts.
