- Description
- North Florida/South Georgia Veterans Health System, VA Medical Center= , Gainesville, FL seeks full and open competition for the award of a contract for the delivery of Diagnostic Imaging Services. The solicitation will be conducted in accordance with FAR Part 13.5 test program. The solicitation adheres to the format defined in FAR 12.303 for the acquisition of commercial products and shall be evaluated in accordance with the procedures of FAR Part 13.106. (2) This solicitation is issued as a Request For Quotation RFQ 573-38-02. (3) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 2001-09. (4) NAICS: 621512 with a size standard of $ 10 million. (5) [LINE ITEM ONE]: The contractor shall furnish all necessary equipment, supplies, supervision and qualified competent personnel to perform Diagnostic Imaging services to include routine radiology, mammography, and other specialty services in the treatment of care for eligible veteran beneficiaries as referred and authorized by North Florida/South Georgia Veterans Health System (NF/SGVHS). Services will be performed within the VA policies and procedures of the NF/SGVHS and JCAHO guidelines. All radiology services provided under the contract shall be in compliance with the standards of the American College of Radiology. The contractor shall provide services to Veteran beneficiaries of NF/SGVHS, Department of Veterans Affairs for the period November 1, 2002 - September 30, 2003 with 4 option year renewals. The Government intends to award a single or multiple award, firm fixed price contract for services in accordance with all terms, conditions, provisions, specifications, and schedules herein, pending the availability of funds. The Authority for this procurement extends from Public Law 104-262 and Title 38 U.S.C. Sections 8153. Services under the contract shall support the needs of the Community Based Outpatient Clinics located in Ocala, Inverness, St. Augustine, Florida and Valdosta, Georgia. Mammography Services under the contract shall include the same CBOC=92s and will also include the needs of the Satellite Outpatient Centers located in Jacksonville and Daytona Beach Florida. Each offer should contain the offeror's best terms from a cost/price and technical standpoint, as contract award may be based on initial offers received, without discussions. The solicitation will issue on or about October 4, 2002. The closing date for submission of proposals is October 18, 2002 at 3:00pm local time. Requests for copies of this solicitation must be in writing and should be received by the issuing agency no later than 7 calendar days from the date this notice is published. No telephone or fax requests will be accepted. Hard copies of this solicitation will be issued to the first 10 requests.
