Description
SUBJECT: Q--UROLOGIST ONE DAY PER WEEK AT VAMC ALEXANDRIA LA VA Medical Center Alexandria LA plans to negotiate a sole source procurement with the firm of Meek, Templeton and Richey, LLC, in Alexandria Louisiana to provide the services of a Board Certified Urologist to perform urology services _ primarily cystoscopies and prostate biopsies every Thursday. Approximately twice per year an emergency need arises requiring the urologist to be on station within thirty-minutes of a request for assistance.There is no other known source to meet the time requirement. Solicitation number is 502-10-03 and is issued unrestricted. The services are required to assist the staff urologist to reduce waiting time for veteran referrals. Any responsible source may request the solicitation and will be considered. The North American Industry Classification System Code is 621111 with $5.0 million annual sales limitation for small business. VA intends to award a firm-fixed price, non-personal services contract for Health Care Resources in accordance with Public Law 104-262 under the authority of Title 38 U.S.C. 8153. Required services include outpatient clinic services while the VA staff urologist performs surgery. Contract term is twelve (12) months with a possible six (6) month extension. The Urologist shall be skilled, knowledgeable, and experienced in all facets of adult Genito-Urinary care as established by industry and VA standards. He/She shall be in good standing with the profession with knowledge and abilities current to equipment, medication and techniques. The Board Certified/Board Eligible Urologist shall provide comprehensive health services relating to all phases of Urological complaints and diseases, routine and malignant. The physician shall perform ambulatory surgical procedures to include, but not limited to cystoscopies, stent placement, urethral dilatations, and transrectal needle biopsies of prostate. There is no call back or overtime included except for emergency surgical assistance. Services shall be provided at VA Medical Center, 2495 Shreveport Highway, Building 7, Urology Clinic, Alexandria LA. Routine workload is approximately 30 patients per day. Cystoscopies average approximately 15 per month, 20 prostate biopsies and one cystometrogram each month. Documentation, shall be performed in accordance with all VAMC established principles, ethics, standards, rules and regulations as set forth in VHA Directives, Medical Staff Bylaws, Medical Center Memorandums, AMA, and JCAHO standards. The primary applicable are 99201, 99202, 99203, 99211, 99212, 99213, 55700, 5200, 52005, 52310, 52281, and 52332. Preferable method of reimbursement is per CPT code. Qualifications: The Contract Urologist shall have an active, current, unchallenged and unrestricted license in a state, territory, or commonwealth of the United States or District of Columbia and shall be board certified. He/She shall speak, understand, read and write the English language fluently; be computer literate and familiar with VA_s electronic Clinical Patient Record System (CPRS)to input and access documentation. The contractor shall maintain malpractice professional liability insurance in an amount not less than $1,000,000 per occurrence for the entire contract period.
