- Description
- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This purchase request (PR) will be issued as a request for quotation (RFQ) under the following F5LSDS21430400. . (iii) This PR and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2001-09. (iv) This acquisition is set-aside 100% for small businesses in regards to size standards. The applicable NAICS is 444190, and the small business size standard is 100 employees. (v) This RFQ contains (16) line item(s): 0001, Charcoal Pallet Filters, 160 EA; 0002, 24 x 24 95% Packs, 64 EA; 0003, Tri Cube 2 Pocket, 80 EA; 0004, Tri Dek 3 Ply Panels Intake, 60 EA; 0005, Tri Dek 3 Ply Panels, 320 EA; 0006, Repipe Breathing Air Filters, 2 EA; 0007, New Manometer, 4 EA; 0008, Hose Rail Systems, 2 EA; 0009, 2-2 Phase Water and Oil Traps w/Regulators, 2 EA; 0010, Fluorescent Bulbs, 128 EA; 0011, New Mechanical Door Latches, 2 EA; 0012, Replace Motor Drive Belts, 6 EA; 0013, Replacement of All Necessary Door Seals, 3 EA; 0014, Replace Breathing Air Filters, 3 EA; 0015, Labor Charge, 1 JB; 0016, Freight Charge, 1 EA. (vi) Vendor shall provide all facilities, labor, transportation, and supplies. Labor includes: Remove intake filters, clean and recoat with tacky filter coating, Install fluorescent bulbs and check for working ballasts, Installation of all filters, Repipe breathing air filter away form the door, Replace breathing air filters, Setting of correct positive pressure, Installation of 4 new manometers, one each filter bank, Check exhaust gasses to meet EPA requirements, recheck exhaust booth positive pressure after 10 working days, Install air hose rail systems. Note: cleaning of filter bank to be done by Hickam AFB (vii) The performance period will be 45 days ARO. Delivery location/Place of performance is Bldg. 1055, Hickam AFB, HI 96853. Acceptance will be performed by the Government. The FOB Point is destination. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) Offers will be evaluated using the following evaluation factors: price and past performance. The contract will be awarded to the offeror that provides the best value to the government. (x) All offers must include a completed copy of the provision at 52.212-3 , Offeror Representations and Certifications -- Commercial Items (Alt I). A copy of the provision may be downloaded from the web-site stated in paragraph (xiii).
