Description
This notice is provided on a competitive basis, with intent to award a firm-fixed price order for Transient Symptomatic Event Detection monitoring services. The quantity for Items 0001 through 0010 is 490 each. Item 0001 Transient Symptomatic Event Detection monitoring service with internet access for cardiologist to view urgent and emergent patient arrhythmias to include patient monitors on an as needed basis. Item 0002 Contractor shall provide Looping and Event monitors. Item 0003 Contractor shall provide unlimited event monitoring of symptomatic patient transmissions on a 24/7 basis for up to 30 days per patient. Item 0004 Contractor will provide patient home enrollment whereas upon request of a cardiologist, a Loop event monitor will be sent to the patients home and the patient will be called with instructions for monitor operation and hook up to include baseline rhythm recording/transmission. Item 0005 Contractor shall be responsible for retrieving the monitor from the patient at the end of the service period. Item 0006 Contractor shall provide access to NMCSD cardiologist for patient test results, both historic and recent to the ordering physician via Internet on a web based management database. Item 0007 Contractor shall supply NMCSD with additional six (6) monitors for in-clinic hook ups and will send one for one replacements upon enrollment of these patients. Item 0008 Contractor will provide in-service training to cardiology staff and doctors on monitor hook up and operation to include the contractors Internet patient management database. Item 0009 Contractor agrees to hold NMCSD harmless from any and all losses, costs, damages, liabilities and expenses from any claims from third parties resulting from contractor?s product. Item 0010 Contractor agrees that NMCSD will be released from all responsibility and liability for monitors that are lost, stolen or kept by the patient. This is a combined synopsis/solicitation for commercial services, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00250-02-T-0147, is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 2001-07. The performance period is 01 October 2002 through 30 September 2003. This acquisition incorporates the following FAR clauses: 52.212-1 INSTRUCTIONS TO OFFERORS-COMMERCIAL ITEMS (Oct 95); FAR 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS-COMMERICAL ITEMS (Oct 95) must be completed and submitted with your quote (please contact Phyllis Kennedy at 619-532-8100 if a copy is needed); in accordance with FAR 52.212-2 EVALUATION-COMMERCIAL ITEMS: Evaluation Factors, in order of importance, are: Technical Capability, Past Performance and Price. You are required to submit a technical proposal showing your companies technical capability to provide the services listed above. Please provide three (3) references with your proposal. These references should include most recent work performed similar to the services NMCSD is requesting.
