Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotation being requested and a written solicitation will not be issued. Solicitation F04626-02-Q-0020 is being issued as a Request for Quote (RFQ). Vendor shall provide all tools, parts, installation, materials, equipment, and shipping necessary to provide the following blood bank reagents: 1) MTS Anti G Card-QTY 4 boxes per month (total 48 boxes for 1 year) 2) MTS A/B/D Monoclonal and reverse grouping gel card-QTY 20 boxes per month (total 240 boxes for 1 year) 3) MTS Diluents 2-QTY 1 box per month (total 12 boxes for 1 year) 4) MTS Diluents 2 Plus-QTY 2 boxes per month (total 24 boxes for 1 year) 5) MTS ID Tips (bulk)-QTY 4 boxes per month (total 48 boxes for 1 year) 6) 0.8% Surgiscreen-QTY 2 boxes per month (total 24 boxes for 1 year) 7) 0.8% Panel A-QTY 1 boxes per month (total 12 boxes for 1 year) 8) Affirmagen, A1B cells for ABO-QTY 4 boxes per month (total 48 boxes for 1 year) 9) Confidence System-QTY 1 boxes per month (total 12 boxes for 1 year) **OR EQUAL ACCEPTED** **NOTE-Items are to be used on the Ortho ID-MTS GEL TEST analyzer and offers must include proof of compatibility** This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 200108 and DCN 20020730. The proposed solicitation listed here is unrestricted. This acquisition falls under the Standard Industrial Classification number 2835 and the North American Industry Classification System 325413. Effective 1 Jun 98, all contractors must be registered in Central Contractor Registration (CCR) to receive an award from Travis AFB. Call 888-227-2423 or www.ccr.gov to apply. FAR clause 52.212-2, Evaluation-Commercial Item applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is conforming to the solicitation and will be most advantageous to the Government. Price, compatibility with existing equipment of the product offered to meet the Government requirement, and past performance are the evaluation factors. The following additional FAR provisions and clauses do apply: FAR 52.212-4 is incorporated by reference and will be incorporated in the Purchase Order by reference. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision completed will not be considered. FAR 252.225-7001, Buy American Act And Balance of Payments Program applies to this acquisition.
