Description
This is a combined synopsis/solicitation for commmercial services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number F05611-02-Q-0702, in accordance with FAR Parts 12 & 13. The North American Industrial Classification System (NAICS)number is 813110 and the business size standard is 500 employees. The proposed acquisition is reserved for small business. LINE ITEM 0001: Catholic Director of Religious Education 1 Oct 02 through 30 Sep 03. LINE ITEM 0002: Option year one. LINE ITEM 0003: Option year two. LINE ITEM 0004: Option year three. LINE ITEM 0005: Option year four. The FOB for this RFQ is Destination. Quatations received with other than FOB Destination pricing will not be considered. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Comercial Items, applies to this acquisition and on addenda applies. The provisions at FAR 52.212-2, Evaluation Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsive, responsible offeror whose technically acceptable offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The Clause 52.212-3, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this acquisition, no addenda applies. Additional FAR clauses cited in the clause applicable to this acquisition are: (b) 12,13,14,15,24. The Clause 52.217-8, Option to Extend Services applies to this acquisition. The Clause 52.217-9, Option to Extend the Terms of the Contract applies to this acquisition. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Status of Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this acquisition with the inclucion of the following DFARS: 252.204-7004, Required Central Contractor Registration, 252.225-7001, Buy American Act and Balance of Payments Program. CATHOLIC DIRECTOR OF RELIGIOUS EDUCATION STATEMENT OF WORK PURPOSE The Catholic Director of Religious Education (DRE) (contractor) is responsible for developing, implementing, coordinating, and evaluating appropriate religious education opportunities for families attending the Catholic Community Center Chapel, USAF Academy, CO. PRE-QUALIFICATIONS: 1. Individual must maintain a lifestyle consistent with Christian principles and the purpose of the Air Force Chaplaincy. 2. Must be able to work harmoniously with chapel staff, other contractors, and volunteers in the chapel program. 3. Must be able to work in the ecumenical and interdenominational environment of the military chapel program. 4. Must possess a Bachelors degree in Religious Education or a minimum of five years experience as a catechist in a chapel or church setting. 5. Must possess a minimum of three years experience working with Catholic Children?s worship or other children?s group activities. 6. Must be a member in good standing with the Catholic Church. 7. Must be knowledgeable on current trends of the Catholic Church and the local diocese. 8. Must have superior organizational and administrative skills. 9. Knowledge of Windows 2000 operating system, Word, Excel and Access programs is desirable. 10. Must furnish a resume as part of proposal showing education, experience, and personal and professional references. The resume or cover letter must address all areas of the statement of work. References will be must include a name and phone number. 11. A background investigation conducted in accordance with DOD Instruction 1402.5, Criminal History Background Checks on Individuals in Child Care Services will be accomplished on the individual selected for this contract. Refusal to consent to the background investigation will result in immediate termination of the contract. GOAL The goal of all activities is to facilitate Christian growth, commitment, and experience in the Community Center Chapel through religious education. TIME/LOCATION/FREQUENCY OF SERVICES 1. The Contractor will provide a graded CCD program from preschool through adult, each Sunday, of approximately 1 hour in length, from September to May. 2. The Catholic Chaplain will determine the location and time of CCD and provide this information to the contractor. 3. Other program times, locations and frequencies will be determined by season and needs assessment. 4. Maintain regular scheduled office hours of not less than 18 hours per week Monday through Friday. Office hours must be coordinated with the Catholic Chaplain. Contractor will be present one half hour prior to and during religious education programs. Advance notice of extra hours will be provided to the contractor at least 24 hours prior to required additional hours. Additional hours outside the chapel are included in the monthly amount paid to contractor. RESPONSIBILITIES 1. The contractor will perform other services in order to ensure the quality of the Catholic religious education program. These services include: a. Planning and coordinating the graded CCD program each week. b. Recruiting, training, and recognizing catechists, catechist?s assistants, substitutes, and other CCD volunteers. Training includes semi-annual in-service classes and introductory meeting at beginning of school year. c. Preparing an annual religious education calendar for the annual chapel planning conference. The calendar should include all special religious education events for the entire year such as training sessions, workshops, socials, appreciations and graduation. d. Preparing the annual Catholic religious education chaplain fund budget for submission in June, taking into account all requirements for appropriated as well as non-appropriated chaplain fund support. e. Accomplishing and monitoring the annual curriculum order for CCD and Ecumenical Vacation Bible School and ensuring necessary materials are available for the catechists with coordination of the Catholic Chaplain and the NCOIC, Community Center Chapel. f. Monitoring all teachers, ensuring they are competent, assist in lesson planning, and supply needs. g. Planning teacher appreciation program to include purchase of appreciation and Christmas gifts. h. Accomplishing all clerical work relating to the religious education and special programs mentioned in this document. i. Registering all students and keep updated files of student attendance. Contact CCD parents regarding chronic absentees or discipline problems. Track weekly attendance for CCD. j. Open and set up classrooms in Fellowship Hall, Roden Room, Youth Center and Education Center. k. Providing resource, scheduling, training, recruitment, attendance tracking, and curriculum support for: 1) Catholic SPIRE (Special Programs in Religious Education) 2) Weekday bible studies 3) Adult Sharing 4) Children?s Church 5) Additional programs as determined by the Catholic Chaplain l. Inventorying and stocking all Catholic RE classrooms and the Catholic RE supply cabinets. Submitting requests to procure all supplies and equipment required supporting the Religious Education program. Fund requests will be submitted in accordance with chapel operating instructions provided to the contractor. 2. The contractor plans, coordinates, and conducts with the Protestant Programs Coordinator all aspects of a summer Ecumenical Vacation Bible School with appropriate classes and materials. Coordinates with the Wing Chaplain or his/her designated representative on facility, funds and curriculum requests. 3. The contractor will work with the NCOIC, Community Center Chapel to ensure that all information is obtained and submitted for background investigations of all personnel working with chapel children and youth. 4. The contractor will plan, coordinate, schedule, and conduct the following special programs: a. 1st Communion, 1st Reconciliation and Confirmation potlucks and receptions b. Children?s Christmas Program 5. The contractor will attend all Chapel Staff meetings, weekly meeting with Catholic Chaplain, and Catholic Advisory Council meetings as needed by the Catholic Chaplain. 6. The contractor will provide resources, scheduling, training, recruitment, attendance tracking and curriculum support for the following sacramental programs. a. First Reconciliation and First Communion for second graders and beyond b. Confirmation classes held separate from the regular religious education sessions c. Rite of Christian Initiation for Adults (RCIA) 7. The contractor will keep updated on current trends of the Catholic Church and local diocese. Attends workshops at Catholic Chaplain Fund expense in order to ensure currency. The Catholic Chaplain will determine applicable workshops. 8. The contractor may use chapel owned or managed audio/visual and Information Technology equipment located in the chapel. The contractor is responsible for returning equipment, supplies, and facilities to the location and/or condition received. Any maintenance needs or damage reports must be submitted to the NCOIC. 9. The contractor will ensure that facilities used for religious education events are clean and secure, to include all doors and windows, following the conclusion of Sunday School and other meetings at the Youth Center, Education Office, Airmen Leadership School, Base Auditorium or chapel. 10. The DRE personally performs all services or will cause them to be performed by a substitute of equal or higher proficiency. The contractor will provide 24 hours notice of the need for a substitute, except in emergency situations. The contractor will provide a list of appropriate substitutes with resume, name and phone numbers at the beginning of the contract period to the Catholic Chaplain. The Catholic Chaplain will notify the contractor as to who is considered acceptable as a substitute. Any payment required by the substitute will be the responsibility of the contractor. 11. The contractor agrees to conduct himself/herself with due regard to public conventions and morals, and further agrees not to do or commit any act or thing that will reasonably tend to degrade him/her or to bring him/her or the Air Force Chaplaincy into public hatred, contempt, scorn, or ridicule, or that will reasonably tend to shock, insult or offend the community or offend public morals or decency, or to prejudice the Air Force Chaplaincy. Arrested or violation of any applicable Air Force Instruction may be grounds for immediate termination. 12. Payment shall be made monthly, in arrears, upon submission of a proper invoice. Invoices will be submitted to the Catholic Chaplain for certification prior to submission to the NCOIC of Financial Management.
