- Description
- This is a combined synopsis/solicitation for commercial items prepared In accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 69D-354-02 and is a Request for Quotation (RFQ). The solicitation, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-09. The NAICS code associated with this requirement is 513220 and the size standard is 12.5 million (can not make over within last 3 fiscal years). The location is VA Medical Center North Chicago IL and FOB is destination. The Statement of Work is as follows: Install Direct TV in specified "dayrooms" only. It will require the installation of separate dish antennas per building and the installation of new cable from each dish to each "dayroom". The following applies: 7 separate buildings, 20 total drops, Total Choice Package and vendor installation. Breakdown by building: Buildings: #7-2 drops, #11-1 drop, #46-1 drop, #66-1 drop, #131-7 drops, #133-2 drops and #134-6 drops. The following are the channels to received per dish antenna: Channel 14-A&E, Channel 15-CNN, Channel 16-Discovery, Channel 17-Disney, Channel 18-ESPN, Channel 19 ESPN 2, Channel 20-ESPN Classics, Channel 21-History, Channel 22-Nick at Nite, Channel 23-Learning, Channel 24-TNT, Channel 25-TBS, Channel 26-USA Network, Channel 27-AMC, Channel 28-Animal Planet, Channel 29-Fox Sports Net, Channel 30-Lifetime, Channel 31-TCM, Channel 32-TV Land, Channel 33-WGN, and Channel 34-Weather. This procurement is being conducted under the Subpart 13.5 test program where simplified acquisition procedures apply and the conduct of the procurement will be in accordance with Parts 12, 13 or 15 of the FAR or some combination thereof. Contractor will contact the Bio-Med Department to arrange for rooms to be available for installation of dish antennas. The following provisions and clauses shall apply to this solicitation: 52.252-2 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request the Contracting Officer will make the full text available. Include a copy of the Representations & Certifications - Commercial Items with offer. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Price 50% Past Performance 25% Technical 25% This solicitation is issued under the authority of Public Law 104-262 and 38 USC 8153.
